*** *** *** (***) insurance plan made a change to their co-pay levels *** went from PCP and Specialist co-pay amount to multiple co-pays depending on what the patient was being seen for by the physician See below a copy of an insurance card with the co-pays listed:See Enclosed ChartNorth Ohio Heart collects co-pays prior to the visit, therefore front office staff does not know how the physician will code the appointment, therefore unable to identify the exact co-pay that will need to be collected.As for the overpayments made by the patient, she will receive a refund of $in the next couple of weeks The reason she did not receive a refund sooner, is due to our policy that if patient is scheduled for another visit within a set timeframe of a paid date of service, we hold the credit to ensure no balance is due on upcoming visit.To avoid future overpayments, we have noted the patient’s account to only collect the value co-pay amount and if insurance determines the higher amount is due, we will bill the patient

We have received your complaint with regards to your dissatisfaction in the level of service you recently received when attempting to refill your - prescription.First and foremost, I would like to formally apologize for your inconvenience. The delay you experienced is not acceptable and does not...

meet our expectations with regards to providing quality patient care.I have reviewed your chart and determined that a 90 day supply of the prescription in question was ultimately sent to the [redacted] pharmacy on Lorain Road on May 29, 2015. I then spoke with Dr. K[redacted] and informed him of your concern as well as your request for a six month supply of  medication. Dr. K[redacted] apologizes for the recent delay as well; however he is unable to prescribe 6 months of medication as you were last seen by him December of 2014. In order to prescribe additional refills, you will need to be seen in the office to be evaluated.All patient complaints are taken seriously and we want to assure you that your concern has been addressed. Our office policy is that prescription refill requests are processed within 24 - 48 hours. I sincerely hope that you have no further delays in the processing of prescription refill requests. However in the event you are not fully satisfied with the service you receive in the future, please feel free to contact me directly at 440-[redacted]

OPERS went from 1 PCP and 1 Specialist co-pay amount to multiple co-pays depending on what the patient was being seen for by the physician. See below a copy of an insurance card with the co-pays listed: See enclosed chartNorth Ohio Heart collects co—pays prior to the visit, therefore front office staff does not know what which diagnosis code the physician will use for the appointment, therefore unable to identify the exact co-pay that will need to be collected.As for diagnosis code, physician selects the code for which best supports his exam and findings, therefore supported in his documentation. This is a medically accepted process and insurance companies, including Medical Mutual of Ohio accepts the process.As for the overpayments made by the patient, she recently received a refund of $80.00.North Ohio Heart does not charge interest on outstanding balances; therefore it is not our policy to reimburse interest.The reason she did not receive a refund sooner, is due to our policy that if patient is scheduled for another visit within a set timeframe of a paid date of service, we hold the credit to ensure no balance is due on upcoming visit. To avoid future overpayrnents, we have noted the patient’s account to only collect the value co-pay amount and if insurance determines the higher amount is due, we will bill the patient.
