September 2014. Please contact your insurance to see where claims for 2015 are being summited from. We have contacted our billing company to discontinue any pre authorizations in behalf of Mrs. [redacted].In regards to x-rays we use Vix Win in our office wich had dates on al| x-rays. There should be no confusion as to when xrays were taken. However as as a courtesy we are refunding her expense with Dr. [redacted] for $81.00. No reimbursement can be done for services already done to her son back in 2014, we have necessary documentation on visits along with consent.

Review: n January 2015 this office submitted a claim for my son's deep cleaning when he never received it. I had made many (four) appointments with Dr. [redacted] all on Fridays a month in advance and his staff called me and cancelled me every time. They would call me and ask me to come through the middle of the day on the day they felt like it, while I was working.

I let his staff know that my son and myself would no longer be receiving service from this dental office we were changing to another dentist. His hygienist had just taken my x-rays and within a month of this I was with another dentist due to all the cancellations. I called Dr. [redacted] office five times if they could please send the copies of my x-rays to my knew dentist. They refused by stating that their computer was not capable of doing this, that they already had done it, that the doctor had not authorized it, that they were too busy, that my file was to big. Finally my husband went by with a thumb drive to get a copy of my recent x-rays. The front desk personnel couldn't do it until Dr. [redacted] come out and copied my x-rays.

Finally after two months of trying to get the x-rays I took them to my new dentist. He was baffled because not only had Dr. [redacted] copied my latest x-rays but ALL my ex-rays from ten years back. He could not tell what was new or old with out having to re-do my x-rays that I had to pay for because my insurance would not cover another set so soon. I called Dr. [redacted] office and let them know that this was really awful for them to do, but since husband was still getting his deep teeth cleaning with them I didn't pursue it I just paid for another set. I just received an e-mail that my dental claim has been submitted, since I have not been recently to the dentist recently. I called [redacted] insurance and they told me that Dr. [redacted] had submitted on Dec 1st, 2015 an estimated for five fillings to perform on me.Desired Settlement: I would like for Dr. [redacted] to reimburse the insurance or me for the last x-rays he charge for me, and for the claim of the deep cleaning for my son. I feel it's only fair I am not asking for all the cancellations he subject me, since I had to take time off my leave and he would cancel the day of sometimes that cost me my leave which is part of my pay.
