Continue to receive Request for Payment on a bill that has already been satisfied by my health insurance, Network Health Plan.
I continue to receive threatening phone calls and Requests for Payment to my home address on a bill that was been satisfied by my health insurance, Network Health Plan. My health insurance paid all of the claims on the date listed on this Request for Payment, which is 10/05/2018. I received many lab tests that day, all of which my health insurance paid. Pathology LLC continues to try to receive payment on claims that have already been satisfied by my insurance. On June 8, 2019 I called Pathology LLC at the Billing Office 866-759-4528. They stated that this Request for Payment was in error, and to disregard the statement. Now I am receiving the same request 6 months later. I have called Pathology LLC daily since receiving this Request for Payment in the mail multiple times a week but they do not answer. I have disputed this Request for Payment via email and in writing via the US Mail. The only communication I receive in return is continuous and harassing Reqeusts for Payment that threaten my credit score and credit report. My Account number listed on said Request for Payment is ***1
The request is for $75.00 for a service rendered 10/05/2018

RE: (External Email) Re: North Shore Pathology: Complaint *** We reviewed Ms. account. It appears we billed the insurance that was listed on Ms. account, Network Health Plan. The claim was denied. We sent the patient 3 statements. Since the account went unpaid, we sent the account to collections. To ensure we’ve done our due diligence, we contacted Network Health Plan to gather more information about the claim denial. We learned that Ms. plan was not a Network Health Plan, but an MHS Health Wisconsin Plan. Ms. claims were denied because we had the incorrect payor on file. Ms. Beard was in fact covered under the MHS Health Wisconsin plan from 12/07/2017-06/30/2019. Resolution:We removed Ms. account from collections. The account on her credit file will be removed as well. Ms. Beard will receive a letter from OAC collections notifying her that she has a $0 balance and that any marks on her credit file from this incident will be removed. We apologize for the inconvenience this has caused Ms. Beard. Please let me know if you have any questions. Thank you,Client Management
