Euclid Family Practice September 13, 2017 Revdex.com Euclid Ave., 4th FL Cleveland, 44115-2408fax#216-861-6365 RE: *** *** Complaint ID *** To Whom It May Concern: This letter serves as a third response to the complaint ID# *** for the patient listed above. *** *** states that his EOB shows we billed for "Iviedical Care" not for a new patient office visit"As mentioned in our previous response, *** *** is getting caught up in language that his insurance company uses and has nothing to do with what we billedThe claim that we filed with his insurance company had no description of the charge listed at all, and only listed the CPT codes (Current Procedural Terminology Codes) of and with each perspective charge, and all the diagnoses codes related to the visit. If *** *** has an issue with the description of the charge "Medical Care", then he needs to take that up solely with his insurance company as they have used this language/description only, and not us. The definition of CPT code as listed in the CPT code book is: Office or other outpatient visit for the evaluation and management of a new patient.*** *** was seen and evaluated as a new patient on date of service 12/06/That is what *** *** was seen for, that is what we billed his insurance company for, that is what *** *** has already paid for, and NO reimbursement will be given. *** *** is more than welcome to complete the Release of information form and that will allow us the share not only the visit notes from that day, but also the claim form that we billed to the insurance company showing no description of charges. filed above in regards to our patient, *** ***, Due to HIPAA, there is certain private health information that cannot be shared without prior consent from the patientBut we will respond to this complaint and address the issues that the patient has already mentioned in his statement. As mentioned by the patient, he was seen at our office on December 8th *** *** was not an established patient with us prior to his December 8th visit, therefore he was considered a new patient and had to establish a relationship with the doctor during this visit, As a new patient, there is a fee for a new patient visitThat fee was billed to *** ***'s insurance company (***), *** received the charge for the new patient office visit and *** processed this charge and allowed $which they considered reasonable and customary and applied that allowable amount to the patient's responsibility to be put towards his annual deductible. This second charge listed on the claim for December 8th was for the earwax removalAs *** *** mentioned ihis complaint, "hundreds of doctors are able to do it with a charge of $22"*** allowed this charge for $which they considered reasonable and customary and applied it to the patient's responsibility as it too was put towards his annual deductible. Both charges total $which is what *** *** was billedThe first statement to patient dated 02/27/did show an incorrect adjustment made on the claimDue to the incorrect adjustment, the patient was initially billed $But all subsequent statements were corrected and showed the proper balance due of $which the patient has since paidBilling notes show patient contacted the billing office once on date 03/13/to bring the adjustment error to the billing office's attentionNo other communication has been made on the patient's behalf since then. After this detailed explanation of charges, if *** *** is still disputing the charge of $116.59, we ask that he take this dispute up with his insurance company as *** has decided what the patient's responsibility is and not the provider of serviceTherefore, no refund/settlement will be made to the patient. We would also like to mention that with any health care provider, there will always be a new patient office visit charge when there is no established relationship between provider and patient.
