Ferrell Duncan Clinic has sent statements to this patient on 04/13/15, 05/11/15, 06/26/15/07/20/and 08/19/We have sent an
itemized statement to the patient Certified Mail in March to the same address and he signed for it in March so the address is correctWe sent another itemized statement to the patient on 09/09/We cannot however, continue to bear the cost of sending every statement via certified mailWe have also called the patient on 06/26/15; 07/18/15; 08/03/15; 08/19/and 09/04/Every time the patient states he has not received statements and the address is verifiedHe is able to receive the statements when certified at the same address but not without being certifiedI believe the patient needs to visit with the Postal Service if that is the caseI believe we did send a response to the Revdex.com back in March regarding this same individual with the same informationThe patient has NOT been turned to a collection agencyThe calls are coming from a contracted "early out" vendor who mails our statements and follows up with patientsWe will AGAIN send the patient a certified mail statementPlease let me know if any additional information is neededThank you

the physician office, the charge was entered in error. The physician did assist the other physician during this procedure but that is not unexpected and there is normally not a charge. In this instance as charge was inadvertently entered, rather than posting the charge for tracking purposes with a zero charge amount. The insurance is being notified of the error and the patient's prepayment will be transferred to any outstanding balances and the remainder refunded. The patient has been made aware of this information and is satisfied.
