Revdex.com 8020 S County Rd 5, Ste 100 Fort Collins, CO 80528 ID: [redacted] I have attached the notes and the ledger showing all of the previous cancelled appointments. Our policy for the changing appointments is three business days (that is our offices' working business...

days). I have also attached an appointment card and financial page that he had signed with that information [redacted] had been reminded about our policy on multiple occasions.  Our policy is on all of our documentation, we have a sign at the reception area and on our voicemail. As you can see on page two the note from the hygienists' conversation with the patient on Monday 7/6/2015 about the sub hygienist for his appointment on Thursday 7/30/2015. [redacted] is again presenting dishonest information. At that time he didn't want to reschedule his appointment and the hygienist reminded him again of our policy.  He stated "We always charge him anyway". You can see on his ledger in less than one year the-patient had cancelled three appointments and our office did not charge him up until 7/28/2015 when he left a message that he wasn't going to come in for his 7/30/2015 appointment. On page one, 9/17/2015, is a summary of the interaction with the patient at check-out. He was yelling at the front desk in such a threatening manner that another patient that was also at the front desk checking out was scared. I had asked the other patient to come around to my desk and her comment to me was, "Thank you for letting me come back here, I am afraid that he is going to pull out a gun and start shooting!" After [redacted] had left the building she made the comment again on how she was afraid to leave if he was still out in the parking lot. Three staff members; [redacted]- the front office staff that he was checking out with, [redacted]-sitting at the front desk, Dr. [redacted], a female patient and myself all witnessed his behavior. He told [redacted] a couple of times that he wanted to talk to Dr. [redacted] because she wouldn't charge him. I asked the patient, on Dr. [redacted]' request, if he could please come with me and have a seat in the consult room so Dr. [redacted] could come in and talk with him, he told me that we wouldn't follow me anywhere. [redacted] stayed very calm with the patient which seemed to get him more mad and frustrated with her. She just kept responding with "Ok!" and he yelled at her, "NO, it's not ok!"To address a couple other areas in his complaint: 1) He said that he had prepaid for a surgery which is not true. If you look through the ledger you can see that in all his years in the practice the patient never had a credit on his account. 2) He said that he always paid his payment on time. Our policy is payment at time of service which he followed until his 7/8/2014 appointment. We again explained to him that we need payment at time of service, he would get upset with us and tell us that he is good for it and always pays his bills on time. Note that it would take him two to five months to pay his account balance. This is why on his 9/17/2015 appointment Dr. [redacted] said that she would like him to prepay for all appointments prior to us scheduling future appointments. We, as a practice, have the right to charge a patient for copy and mailing fees of all of their records. When a patient pays for x-rays they are paying for the professional to read and diagnose those x-rays. Dr. [redacted] did not want the, patient to come to the office due to his behavior at the time of his last visit and two voicemails that he left yelling and threatening the office. We still have the voicemails saved on our system, which we would be happy to share with you. In summary, we do not wish to write off the fees and if anyone owes anyone an apology it should be him in the way he treated the staff and scared a patient. If you need a statement from the patient I am sure she would be happy to oblige. I have used this periodontal office for 15 years, but recently left their care after what I considered an inappropriate professional communication. I had to cancel a procedure with 24 hours notice when my mother was critically ill, and following her death, I received a certified letter from the practice, asking that I pre-pay all procedures and submit to a policy of 5-days notice on all scheduling changes. The staff is terse when you are in their care, [redacted] seems over-scheduled, and therefore overly rushed, and the care has declined steadily in recent years, but this letter is the last straw for me. Life is too short, I will be seeking care elsewhere.
