I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below I never received any paperwork to assign benefits from [redacted] to Northern Radiology I assumed all paperwork was completed at the hospital and that the paperwork would be shared by all parties involved A few things that Northern Radiology can do: do not wait months to start collecting payment on services rendered Send all request for information and other important paperwork return receipt requested or certified to verify receipt, have their paperwork at the hospital also for these instances Regards, [redacted] ***

To Whom It May Concern:I am writing in response to the complaint filed by Mr*** ***Mr*** received radiology serviceswhile being treated as a patient at *** *** HospitalNorthern Radiology Associates provides allprofessional diagnostic imaging services to *** *** Hospital, and
thereby collects insuranceinformation from the hospital after services have been provided.When Mr*** originally received his services, *** *** Hospital had no insurance information onfile for Mr***, so we sent the patient a bill with the expectation that Mr*** would call in with hisinsurance informationOn July 6, 2017, we received a call from *** that they had received astatement from Mr***, and they asked us to submit the claim to themWe submitted the claim to*** and received a denial for no Assignment of Benefits on fileWe mailed the Assignment of Benefitspaper work to Mr*** on 08/14/17, however it was never completed and mailed back*** sent twomore denials, one on 08/10/17, and another on 09/14/because the Assignment of Benefits was nevercompleted by the patientBecause the patient did not complete his paper work, his claims could not beprocessed, and we left the patient's charges as his responsibilityThe patient contacted our office againon 10/31/stating that he had received another bill, but he had not received an AOBThis wasmailed out again to the patient on that date. Our office did finally receive an AOB from Mr*** *** on 11/15/2017, and we have sent his claimsback out to *** for processingOur office makes every reasonable effort to process patients' claims, however we must rely on patientsto complete and return all necessary forms in a timely fashion to avoid timely filing restrictions.Please find attached all documentation received for Mr***If you have any further questionsregarding this claim, please contact me directly at ###-###-####.
