The information provided in this report is the result of an inquiry Into the referenced complaint from [redacted]. In response to your request on June 17th for your complaint received on June 4th along with receiving our own request for the same information from [redacted] directly, the requested information was mailed out on June 10th. In an attempt to make sure [redacted] received the intended information, we called to verify her receipt. She stated over the phone that indeed she did receive and stated that she did not have any additional needs at this time. We also believe there may have been a misunderstanding on the form she would have completed back in December as she referenced in her claim. The form she would have received would have been stating the process of notification of the clinic management switching to a new vendor and that if medical records needed sent over to the new clinic manager she would need to sign a release and have it sent on to us. Perhaps she returned it to the new clinic management company. Never the less, I believe she has received the requested information and hope the issue has been resolved. Thanks!
