Complaint: [redacted] I am rejecting this response because: There was a copy recent court order September 2,Hawaii Behavior Health stuff did call her one time asked for divorce decree that was not different from court order included in the letterI called vice president of Hawaii Behavior Health and leaved several massage they did not even answer itUn-profesionalism of Hawaii Behavior stuff it appear disrespect other parent (mother) and discrimination base on their action and denied and disrespect court order Sincerely, [redacted]

Thank
you for the document (divorce decree) that was received 03/11/16. We were
not able to send out any forms until we had this on file due to this decree
stating parents’ custody agreement and the court order citing a decree we did
not have on file.
We’ve
been trying to follow up with mother on setting up a date for her to come and
review the documents. Parent was called twice on Friday, 03/11/2016, and once on
Monday morning, 03/14/2016. Messages were left for mother, and we await her reply

There was a request to obtain information by mother of
client. Office staff called mother on Friday,
February 5th at 9:03AMStaff informed mother:
1) She was
not listed on any consent for exchange/release of information forms
that we
have on file. 2) The
request she sent cited the divorce decree. The divorce decree stated she
had joint custody. Due to there not being a divorce decree on file, we
asked for a copy to confirm this.
The second call to mother was made on Wednesday, February 10,
2016. There was no answer Staff left mother a voice message
stating that this was a follow up call to obtain a copy of the divorce decree
At the end of the message, staff left a contact phone number so mother could
call back when she received the message. There was no call back or voice
message noted from her, nor was a divorce decree received
Due to confidentiality laws, we are not allowed to release
information without proper documentation. There is also a recent court
matter regarding this client. Uncertain if documents regarding this court
matter will be needed as well
