uninsured for dates of service of 6/25/15-6/25/16, however, there no uninsured dates that qualified for Financial Assistance at that time. The guarantor was sent a letter dated 6/25/15 explaining that if there are services during this date range that do qualify for assistance, they will be processed according to the policy guidelines. Both the Guarantor and her child were insured for the dates of service in question. Please let me know if you need any documentation. Thank You
