I have reviewed the response made by the business in reference to complaint ID ***, and have determined that this does not resolve my complaint. For your reference, details of the offer I reviewed appear belowI never received any statements from this business for any amount dueIf I had received a bill I would have paid it promptly like I pay all of my billsI never received a phone call from Oak Leaf Family dental eitherI don't know where the statements went that they said they sent but they did not come to my residenceI moved in April of but I had my mail forwardedI have never once had a problem with receiving mail at either my apartment that I lived at at the time I went to Oak leaf family dental or at my new addressIf they attended to call or send me bills they are sending them to the wrong address and also have the wrong phone number.
Ever since I found out about this over due balance from checking my credit report last month I have contacted oak leaf family dental on several account and never receive a phone call backIt seams that nobody can tell me what the charge was for or when it was from and then say that someone will call backI repeated my number to them and still have not received a phone call back.
I called in January to have them send me my dental records because I switched dentists and it took several attempts to finally have them send them months laterAlso not once when I called for my dental records was an outstanding balance mentionedOak leaf family dental has always had bad communication from the first time I went to them.
If I received a bill I would have paid it promptlyThat is the facts and I do bro believe That oak leaf family dental said that they attempted to contact me and sent statement because that is a complete lie.

I write in respond to the complaint from *** ***, in regards to Oak Leaf Family DentalAt Oak Leaf Family Dental, we strive to provide each of our patients with the highest quality health care experience possible and take pride In committing to each patient's oral
health.On June 16,2014, Ms*** presented in our office for an exam and cleaning, which the charges were submitted to her Dental InsuranceAfter the insurance payment was received on August 22, 2014, Ms. *** had a $balance, due to the deductible being applied for preventive services based on her insurance policyOak Leaf Family Dental mailed Ms*** multiple billing statements, beginning on August 26, to the address listed on her patient registration formWe show that statements were mailed to Ms. *** between the dates of August 26, and January 15, (specific dates listed below) along with several attempts to contact the patient by phone in regards to her balanceThe dates of the billing statements mailed include: 8/26/2014, 10/2/2014, 12/01/2014, 1/15/2015, 2/10/2015, 3/16/2015, 05/12/2015, 05/27/2015, 7/20/2015, 10/01/2015, 11/16/2015, 12/11/and 1/15/Phone calls were made on the following dates in regards to Ms*** account balance: 09/09/2014, 10/21/2014,08/03/2015, 10/01/2015, and 2/10/After several attempts to contact patient with no response, we turned the account balance of $over to the Collection AgencyDue to the account being submitted to the Collection Agency, Ms*** is responsible for the collection feesassociated with her account, creating a balance of $We welcome the opportunity to provide you any additional Information needed for your Investigation of this matterWe can be reached directly at 414-304-Sincerely,

I write in response to the complaint from [redacted], in regards to Oak Leaf Family Dental. At Oak leaf Family Dental, We strive to provide each of our patients with the highest quality health careexperience possible and take pride in committing to each patient's oral health.
Ms. [redacted] first came into the practice on June 17, 2014. Ms. [redacted] completed and signed a patient registration form on June 17,2014. On the patient registration form line stating home addressMs. [redacted] lists: 3755 Northway Apt 204, Oak Creek WI, 53154. We received notification on September 9, 2014 from the post office stating the address we had on file was not a true address. We encourageMs. [redacted] to come into Oak Leaf Family Dental to get a copy of the patient registration showing what she listed as her address.
When looking though Ms. [redacted] account I am not finding (413)663-9870 listed anywhere on, file. We have (413)652-2215 listed on all documentation of patients. When Ms. [redacted] called on 06/03/16 sheverified the (413)652-2215 was her correct number.
Ms. [redacted] completed a release of records form on 1/27/2016. On this form Ms. [redacted] listed [redacted], WI 53100 as were she would like her records mailed to. Ms. [redacted] called on2/12/16 and it was noted she wanted her records sent to the address listed on her release of records form in her file. We apologize for the delay In mailing the records following the initial request from Ms.[redacted] but trust that they have been received by her.

The dress listed on my patient registration form was [redacted]. I would not list an address that is wrong. If Oak Leaf Family Dental has always had the correct phone number than why did I never receive a phone call from 2014-2016 stating that my address was incorrect and undeliverable mail was being sent back to the practice? When I called to have my records released It took over a month to actually get my records and I had to call back several times to verify my address [redacted] WI 53110. I have not lived in Oak Creek for over a year and would not request my records be sent to an address that I do not live at. I am sick of having to go back and forth on this matter. I want that $62 that I paid due to Oak Leaf Family Dental because they were mailing an invoice to the wrong address and not pick up the phone and calling me to get my correct address. Oak Leaf Family Dental had my email on file as well because I would get emails to confirm my appointments. Not once did I get an email stating that my address that they had on file was incorrect. If Oak Leaf Family Dental received a notice from the post office that my address was not an address then why would Oak Leaf Family Dental Continue to keep mailing invoices to the same address? I work in an office for an insurance company and when we get mail back because it is undeliverable or get notice from the post office that an address for our customer is incorrect we take every step we can to try to get ahold of the customer to get the correct address. We do EVERYTHING that we can do including calling every number we have on file and emailing the customer. Oak Leaf Family Dental did not try to get ahold of me to tell me they had the wrong address on file or to tell me that I owed $50.00. 
I want my $62.00 back from Oak Leaf Family Dental and I want the collection off my credit report. Oak Leaf Family Dental screwed up and I will not be held responsible for their mistake!
