I completed 20 hours of treatment at October Road, ***. I asked the provider specifically if they were in network with*** as I was searching for a provider that accepted my insurance. They told me they were in network and my payment would be $5 per visit copay. I was told several times that I had a $5 copay and I informed the check in and counselor that my deductible had already been met. I have received a bill from my insurance for $480. I went to the office 10 different times in 2 hr increments. The bill was billed as a lump sum of 480 like I went there one time. It is also not billed correctly. There is no coding or explanation of what the visit was.

Billing Adjustment Please re-file the bill correctly. If this treatment is not covered as I was told at my initial consultation and several times during visits, I will be filing more complaints.
October Road, Inc. is in network with ***. The customer had a co-pay of $5.00 per visit. He also had a deductible of $400.00 and an out of pocket of $800.00. Until these deductibles are met, he is charged the full amount. After his deductibles are met, he would be covered at 100% with a $5.00 co-pay.
Additionally, the customer has an outstanding co-pay bill with October Road, Inc. for $95.00.
It is recommended that customer contact his insurance company to gain a complete understanding of his financial responsibility and pay his outstanding co-pay amount with October Road, Inc.
