Ms*** did pay for four weeks (6-8-to 6-29-15) of Summer Daycamp in advance. The charge in question is not due to Ms*** not filling out or signing a withdrawal form. We did not charge her for any weeks that her child missed completely, even though that is
policy and is stated on our application for enrollment, which Ms*** signed. Her daughter actually attended for part of both the weeks of 7-6-and 7-20-15. When Ms*** was reached and asked us to withdraw her daughter, we did so effective 7-13-and only charged her the week's rate for 7-6-15, even though her daughter also attended part of the week of 7-20-15. In good faith, the charge for the week of 7-20-was removed. Our daycare director informed Ms*** of the one week charge that was due and of the removal of the other week's charge. Since Ms***'s daughter was enrolled as a regular camper, she was charged our regular weekly camp fee for attending part of the week of 7-6-15. We did attempt to contact Ms*** regarding the remaining balance by mail at the same P.OBox shown on her customer information which she supplied the Revdex.com. OCS mailed a statement on 8-19-15, and two letters dated 9-16-and 10-14-letting her know that her account would be referred to collections if she did not contact us. The account was not submitted to collections until 12-10-15. We would have preferred Ms*** to have responded to our efforts to reach her regarding the charge so that we could have resolved it before if was sent to collections. I am attaching the portion of the application for enrollment for summer daycamp that includes the information pertaining to billing and collections signed by Ms***. Due to privacy issues, I cannot send any other portion of the form. Also attached are copies of the collection letters which were mailed to Ms***.Thank you
