***Buyer Beware***
I made three separate purchases from this "company" on 5/16/
1) A Service Dog Kit with expedited shipping (within business days for $extra) for a total amount of $The Service Dog Kit advertised on the website included a Service Dog Vest, which I paid an additional $extra to have my dogs name embroidered, a dog tag (Blue Service Dog Disclaimer) a Universal ID Card, and a Card Holder(As Advertised on the website)
2) A separate transaction which included a $registry fee, to be listed on their Service Dog Registry website, and $for a Service Dog Photo ID, (As Advertised on the website)
3) 3rd transaction for $was for immediate documents, Certificate of Registration, Universal ID, Reasonable Accommodation Certificate to have on hand while I waited for the "official" documents to be received by mail(As Advertised on the website)
I received on the 4th day 5/22/a vest with my dogs name embroideredThe embroidery style does not match what is advertised on the website nor does the patches match; missing a "service dog" patch on the side, there is only one sewn on the topI received the Black Dog Tag Holder and the Blue Service Dog TagThe shipment was missing the Universal ID Card
When I tried to contact this company there was no initial response; you have to leave a message that goes straight to voicemail at the number provided on the FAQ section which is for a RMA #, no other customer service contact information is found anywhere on the websiteI then sent an email to inquire about my missing items; both ID cards and certificate paperwork, and asked about the dog registry and where to locate the information from the OSDR # provided
I received extremely vague responses on where my missing items were, and no clarification on where the registry was located or how to look up my dogs informationThe email exchange became very heated after several attempts to obtain this informationIn addition, the emails received used very poor grammar, I assumed this was outsourced to another country the way the emails were worded, however, it was someone located in AZ
The "gentleman" that called me at 9:EST after I left a very stern 2nd voice mail stating I would contact my credit card company and Revdex.com as well as social media outlets was EXTREMELY RUDEHe used very derogatory terms and stated this is why they do not have a customer service phone numberAfter a very tense phone conversation he stated he would "send me to collections" if I contacted my credit card companyThen he hung up on me
I sent one final email letting him know he has a fraudulent websiteI received my embroidered dog vest from a THIRD PARTY Vendor located in OHI called that company direct from the shipping label and they could not pull up ANY information on my order and, confirming they are a third party vendorThe vest on their website retails for approx$with free shipping
I contacted my credit card company to dispute the $for the missing ID cardsI received the partial service dog kit, although, it is not as advertised, it was outsourced and the company in OH probably does not realized they have a fraudulent wholesaler
The same "gentleman" that called me at 9:EST called back mins later asking for a "***" I told him he had the wrong number, but kept him on the phone and proceeded to ask him questions calmly and stated I a bad exchange earlier with another rep..."still acting like he knew nothing"He confirmed that their main office was in Phoenix, AZ, and they have a manufacturing facility in OHHe said that the calls are routed in rotation, and acted like he didn't just call me earlierAfter asking several questions, he was pressing to get off the phoneHe stated he could not help me because his could not close the customer screen he was on to search for another
My credit card company was able to provide the address that this company uses, and with my own investigation found a completely different company registered to the address which has no affiliation to Service Dog merchandise
As I've said earlier, buyer bewareDO your research fully before making a purchase, scam companies do existI have learned my lesson and so has my wallet, don't let a nice looking website fool you!

I have tried to contact this business via email and the only message I got was an email response saying that all questions are answered on their page online. However the product they sold me for my service dog , but it had the name of another dog on it. It is the same name that they have on their website, which is a template. and not personalized for my dog and my needs. There is no telephone number or address listed on the website to contact anyone directly or respond to bad orders.

I spent $70.00 registering my dog. I received an OSDR number by email. Later I received an unprinted certificate and a generic card, neither of which had my OSDR number, my name, or my dog's name. It was void of any personal information. I have recently come to the conclusion that the number is actually meaningless.

I also spent $159.00 for a kit from their sister company. It was a total rip-off.

The Service Dog Kit advertised on the website included a Service Dog Vest, which I paid an additional $18.00 extra to have my dogs name embroidered, a dog tag (Blue Service Dog Disclaimer) a Universal ID Card, and a Card Holder. (As Advertised on the website)

2) A separate transaction which included a $35.00 registry fee, to be listed on their Service Dog Registry website, and $55.00 for a Service Dog Photo ID, (As Advertised on the website)

3) 3rd transaction for $37.00 was for immediate documents, Certificate of Registration, Universal ID, Reasonable Accommodation Certificate to have on hand while I waited for the "official" documents to be received by mail. (As Advertised on the website)
