Thanks for reaching out regarding this matterWe believe this has been properly resolved at this point with our member/ customer Mr [redacted] Below you will find a detailed explanation of this situation:Ohio Insurance Services does provide third party administration services for the Wellness Program offered by Mr [redacted] ’s employerThe program provides for reimbursement of in-network approved deductible expenses up to $per member, with an additional $available for Spouses We do ask that claims be submitted in a timely basis and typically provide timely funding for claims received within months of the members date of serviceWe strive to provide timely and accurate claims reimbursement for any group which we administer funding programsMr [redacted] was fully reimbursed for any and all eligible claims as of May 18th, 2016.In reviewing our office records it appears that there were several unique factors that contributed to an extended reimbursement timelineOur office did not record receipt of Mr [redacted] ’s claims reimbursement submission until March 14th, Upon review of the claims which were submitted, those eligible for reimbursement had a date of service of January 13th, which exceeded timeline for standard fundingDue to the lack of timely submission we had to take additional time to review the claims submission, this involved working with the member to better understand the situation before approving the claims for reimbursementSeveral of the claims being submitted were not eligible for funding due to not being approved deductible expenses or being out of networkAfter speaking with the member we were able to send Mr [redacted] a check for 100% reimbursement of eligible claims in the amount of $on May 18th, 2016.As stated previously we strive to mail reimbursement checks to members within business days of our receiving the claims in good orderUnfortunately in this situation there was some confusion regarding the submission and receipt of these specific claimsAlso, the claims were not received in good order due to untimely submission and over half of the claims being submitted were not eligible for fundingWe have been in touch with this member and the reimbursement check was presumably received as it was cashed on June 9th, 2016.Sincerely,Brian PS***Marketing DirectorOHIO INSURANCE SERVICES AGENCY, INC

We have a Wellness program through this company from the City of Steubenville, when I applied for the the wellness reimbursement on Feb I talked with Edith R [redacted] she said they pay their claims in days but I needed prove of medical expenses of $for I advised her that we had only received the single page form for refund and no additional information about the programSo I faxed the medical expenses for to Katelyn S [redacted] and on March 10, , I called her to follow up on it and told her I faxed pages on Feb 25, 2016, she said they were backed up and just getting to those dates.On March I got an email from her saying they received my claim & check will be mailed that morningI contacted her again and on March she emailed me that she had not came across my claim so I emailed it to her that same daySince then I have called her office weekly and bi weekly and she is not in and never gets back in touch with me, I have even asked Lori who answers
