Review: I been talking to chris in business office of [redacted] to get on the charity program she told me with my income would be ok sent chris the papers and copys she wanted now the business office for ohio valley at [redacted] now sent me a final notice I have told them I working with chrisDesired Settlement: to give chris a chance to get me on the charity program like she said so I don't under stand they work for the same hospital

Unfortunately, we could not make that determination due to an outstanding request for information sent to you 4/19/16. Current policy is to continue the statement process while the financial assistance determination is made by OVH.A patient may request an extension if there are extenuating circumstances preventing him from retrieving the required financial information.
