Heather B*** Business Office manager has tried to contact this patient several times to resolve matterUpon review, all claims have been paid according to plan by Cigna and authorization had been obtainedThe balance owed is not due to a precertification issue but is the remaining balance patient
has in order to meet his deductible according to his carrier's plan

I need to know what I am doing regarding the study...in home or lab. I also need to know what I am doing regarding the HCAP...I was approved for both but I am not sure now if that's an issue or if I need to reapply or what

I also returned ***'s calls and was sent to the central scheduleing line I left a message and finally received an email April 11th *** said she would be out of town and that I could reach *** Suzanne H*** I will be sending *** an email today in an attempt to make contact and correct this issue The calls *** left were during my work hours This is still on going and I am getting passed around personnto person after + years It should not take this long to complete a medical procedure

Due to a change in billing companies in 2014, the current billing service company does not show any previous payments. Please work directly with the OHSS Patient Account Resolution Manager at the RBO in Indianapolis. She can be reached at [redacted] (Office).

The patient was seen at consultation and was informed by the physicians’ office that his insurance would cover the visit.  While insurance did cover his consultation visit, the Insurance allowable payment for his overnight study was applied to the patient’s deductible and...

co-insurance. The patient was not aware of the deductible and co-insurance policy coverage for this service.  As a courtesy to the patient, OhioHealth Sleep Services will adjust the billing amount owed and the patient will have a zero balance.Mr. [redacted] was notified that his billing would be adjusted to reflect a zero balance. Thank you. [redacted]

Previous billing office confirmed the balance was patient responsibility with Aetna on 12/29/15, the balance was not moved to patient responsibility, it remained in insurance due. · Our billing office took over patient’s account 1/15/16. The account balance was confirmed on 2/2/2016 with...

Aetna and was moved over to patient responsibility on 2/3/16. · The patients first statement reflecting patient responsibility was sent out on 2/3/16. · We have reached out to the patient 6 times since March of 2016, on 4/13/16, patient was advised of his balance and advised our team that he would be reaching out the insurance company to receive further clarification. · We tried to contact patient a few weeks later on 5/4/16. We have not received any return calls from the patient in regards to his outstanding balance.We are willing to work with this patient regarding this account

I am rejecting this response because: None of the bills sent after 2014 reflected the initial charges nor the payments that have already been made by myself.
