Check was mailed to Bank Lockbox where it was coded by BOK personnel with incorrect Acct number and was then posted to the incorrect acct. Patient did not provide us check information at the time of call with [redacted] in our customer serivce. We have...

identified the error, consulted with lockbox manager at the bank and posted to the patient's account. Collection letter was an inhouse collection letter. Patient is not in a collection status.

THEY DID SEND ME A COLLECTION NOTICE DATED 10/15/2015.IT SAID "COLLECTION NOTICE" AT THE TOP OF THE PAGE. THE LETTER SAID: IF WE DO NOT HEAR FROM YOU WITHIN SEVEN DAYS; YOUR ACCOUNT MAY BE REFERRED TO AN OUTSIDE COLLECTION AGENCY. IF THIS IS'NT A COLLECTION NOTICE I DO NOT KNOW WHAT IS! I PAID THEM ON 09/21/2015.
