Olsen Orthodontics refused to transfer our daughter's complete medical records to her new orthodontist after we moved to another state. When we first contracted with Olsen Orthodontics for our daughter's braces in Aug 2012, we explained that we were moving to [redacted] in Jun 2013. We were told that her braces would be removed between Oct 2013 and Jan 2014 and that we could transfer her to another orthodontist if necessary. Happy with her progress, we decided to stay with Olsen Orthodontist until her braces were removed. For eight months, we drove our daughter back and forth from [redacted] to North Carolina for her monthly appointments. These trips were costly and required her to miss school and one of us to miss 1-2 days of work. After her appointment in Jan 2014, we asked if there were any reason why they hadn't removed her braces when originally plan. The doctor's explanation was that he wanted to continue making adjustments that admittingly were minor and would only be noticeable by another orthodontist. We asked him to remove her braces the following month if that was all he was doing. He agreed and ordered a retainer for our daughter with instructions for two follow-up appointments one month and six months later. We found an orthodontist in [redacted] who would see our daughter for these follow-up appointments. However, when asked to transfer our daughter's complete record to this orthodontist, Olsen Orthodontics refused on the grounds that we "demanded" the removal of her braces too early. Eventually, they agreed to transfer the digital images, but not the treatment record.Desired SettlementTransfer our daughter's complete dental record to the office of Dr. [redacted] Rocky Mount, (XXX) XXX-XXXX.Business Response First, we are very pleased to know that any family/patient would be willing to travel back from a different state to continue their treatment with Olsen Orthodontics. We receive it as a tremendous compliment that their positive experience during treatment helped them make that decision.We understand that family moves are a part of life and we frequently facilitate that process both into and out of our practice. We understand the challenges of moving and so we try to make the orthodontic portion of that transition as easy as possible for our patients. To this same end, the American Association of Orthodontists provides a form entitled "AAO Transfer Form - Patient in Active Treatment", that allows an orthodontist in any state to transfer pertinent information about a patient's treatment. Some of these important pieces of information include the goals of treatment, the plan, and the progress of treatment up until the date of a move. Other extremely helpful information contained on that form allow for the actual physical care to continue under the supervision of a new orthodontist. That valuable information identifies the type of braces and their prescription being used, the size and type of wires, rubber bands, any headgears or other external appliances that have been utilized. This information is very helpful to know for patients in active treatment so that optimal care can continue. For this particular patient the request for transfer information was unique. First, active treatment was complete. This means that braces and wires, etc. had been removed and retainers were now being worn on a full-time basis. Therefore, the form typically recommended by the AAO for "Patients in Active Treatment" was going to provide a significant portion of historic information that was no longer relevant to the patient's stage of care. For that reason, the AAO transfer form was not included with the transfer records (photographs, radiographs) that were sent to the new orthodontist. If the patient or receiving doctor would like to have access to the information that is typically contained on the "Patient in Active Treatment" form, we are more than happy to provide that information. All we would need is to be made aware of the need. We certainly apologize for any misunderstanding that may have occurred during this transfer process. Our relationship with our patients and their families is very important to us and we strive to provide the best care we can. We lament that any misunderstanding with the transfer of information should take away from an otherwise great doctor-patient experience and a beautiful smile. Our solution: Now that we know that additional information is requested, we have gladly completed and will deliver the "AAO Transfer Form" to the new orthodontist along with our visit-by-visit treatment notes.
