I saw this patient on August 14, 2015 with complaints of back pain. On examination of this patient she had diffuse abdominal pain along with low back pain. I worked her up per the standard of care by obtaining x-rays and urine analysis Including a pregnancy test. She was
ultimately diagnosed with low back pain, endometriosis, abdominal pain and ADD. She was told
to take Ambien and Norco as prescribed and to continue taking her Adderall. At no time did she
refuse therapy and in fact gave written consent agreeing to the following treatment.
She was treated appropriately given the appropriate medications and I standby my treatment of this patient. I refuse to do any refunds.

What is being said is absolutely false. The doctor refused to treatment unless I agreed to new xrays even though I had xrays on file from only a few months prior.
