office, we billed her insurance for a medical office visit, visual field and OCT testA claim of $was billed to her primary insurance Medicare PA.2/2/2015- A remittance advice was received from MedicareMedicare allowed $they paid $which left a balance of $This was submitted to her secondary insurance on file. 7/23/2015-We received the explanation of benefits from *** *** PA they denied payment because the patient wasn’t identified. 8/5/2015-We resent the claim to *** *** *** and they denied the claim stating that it may be covered by another insurance.8/28/We called the patient and left a voice mail- We asked them to call us back with the correct insurance informationThe card the patient gave us was for vision and dental coverage not for medical benefitsWe never received a return telephone call.12/25/2015-We billed the patient for remaining balance of $ 1/22/2016-We billed the patient for remaining balance of $44.87. 1/28/2016-The patient called stating that the bill in question was for her visit not They stated that they had made all their paymentsWe tried to explain that the balance was for the visit on 1/13/not a visit in They stated they made their final payments and wasn’t going to pay for those services. 3/10/2016-We reviewed the entire accountThere was an electronic payment error for that visit This decreased her balance $The current balance went from $to $44.88.To this date we still don’t know the patient’s secondary insurance that covers the 20% that Medicare doesn’t coverWe would be happy to submit to that insurance if they would provide us with informationIf you have any question feel free to contact us at ###-###-####Sincerely, Crystal B Practice Administrator
