See attached documentOptimum Sleep Associates, LLCMontgomery RdSuite LCincinnati, Ohio 45236Revdex.com BureauEast Fourth St., Suite 600Cincinnati, Ohio 45202November 10, 2015Re: Customer Complaint ID [redacted] Dear SirS,I am in receipt of customer complaint ID [redacted] I understand her complaint where she states that she has had endless problems with her continuous positive airway device and her mask fit problemsI realize her frustration and am sorry she has had to endure themSeveral clinical notes were made by the nurses and therapists representing her various visits with their attempts to resolve her problemsEvery visit was taken seriously in an attempt to resolve the problem that was defined by the patientAttempts were repeated to fit the patient with the proper sized mask.In addition her continuous positive airway device (the entire machine) was exchanged with a new device during one of her visitsDuring October the patient complained of water being sprayed out from the machineShe was advised to turn down the heat setting on her humidifierThe water was from Condensation from the humidifierI believe that the problem was resolved.On November 6, the patient came into our office aand was refitted with another maskThis mask was the same model of mask that she had had when she was first introduced to Continuous positive airway therapyShe seemed satisfied with the mask as she left from the officeWe will continue to service the patient and her future needs and we will attempt to resolve any issues that she mayencounter.Respectfully,Michael DM***Executive Director
