Good Afternoon,A refund of $was issued to the guarantor [redacted] on 08/09/ The final payment from the patients insurance company came back to us on 08/05/2016, it was processed and a refund was entered to be processed on the same day of 08/05/ The check was then cut on 08/08/by our book keeper and mailed the following day to the patients home address on 08/09/ The delay in insurance payment was due to the fact the dental insurance ( [redacted] ) required a denial of payment from the medical insurance (***) The insurance claim was issued to the medical insurance on 05/07/(the day following the service date) and was received as "denied" on 07/20/ The medical denial was then sent with the claim to the dental insurance ( [redacted] ) for additional processing [redacted] responded stating there was no coverage for the panorex x-ray that was taken due to frequency limitations [redacted] has on how many and how often the patient can have x-rays done The only reason the patient was not refunded more was because the dental insurance ( [redacted] ) denied covering the x-ray 100%.Please do not hesitate to contact our office should you have any further questions or need any other information.Alanna S***

was informed by us and by her insurance company that she was personally responsible for part of her bi|l for services.The treatment that she underwent was elective. She was referred to our practice by another doctor, hence her business was in ho way solicited by us. Prior to undergoing any treatment she was seen by the doctor in consultation. At that time an insurance predetermination was initiated, and she was informed about her financial responsibilities for the proposed treatment before she underwent treatment. In fact, she signed a document that clearly described her financial responsibilities for her treatment.We did not engage in any unfair business practices. The complainant was informed of what her financial responsibilities would be, and she acknowledged as much by signing the document.Please see all attached, requested, supporting documents. Please contact our office with any further questions or concerns.

Review: I went to Oral and Maxillofacial Surgical Associates in November 2012 to get my wisdom teeth out while I was home for break from college. We did the pre cert and went through all of the insurance information so we would knew how much everything would cost. They told us anesthesia would not be covered so we got a private insurance policy that would work best for me so it was affordable. We paid the necessary co pay and did our part. I had the procedure and we were given the okay that everything was good to go and covered. 23 months later after I've graduated college and am now making monthly payments on my student loans I receive a call from Oral and Maxillofacial Surgical Associates saying that they are sorry but my insurance didn't cover my wisdom tooth extraction from two years prior. I had just moved to Colorado that week and was told that I owe $821.68 because their office didn't get my information to my insurance company ([redacted]) in a timely enough fashion so it wasn't covered. I now am recieveing calls from debt collectors such as [redacted] at 7AM that I have debt to be paid to this business. I have always paid my bills on time and pay a big part of my student loans every month, but I do not find this situation just in any way. If I would have known this procedure would not be covered I would have found another way to cover it or gone through a different provider, but I was not given this imformation until almost two years after the procedure and after moving out of the state. I'm also afraid that their mishap could negatively affect my credit.Desired Settlement: I don't feel I should pay the $821.68 because it was Oral and Maxillofacial Surgical Associates fault for not doing their job. I don't want this situation to hurt my credit.
