Last night I checked online for a dental claim submitted from DrS [redacted] for the 12/2/removal of tads - there is no claim Today I called the dental insurance company to double check to see if there was a claim from DrS [redacted] that was not showing up online The dental insurance company representative said there is no claim from DrS [redacted] She said again that I must have a procedure code in order to submit a claim Since DrS [redacted] is out of network and I am not gettting assistance from the Revdex.com in getting a claim that I can submit, she suggested filing a complaint with the American Dental Association.The form that DrS [redacted] 's billing person gave my husband does NOT have a procedure code.Last night I also checked online for a medical claim submitted from DrS [redacted] - there is no claim Today I called the medical insurance company to see if there was a claim pending but not showing up from DrS [redacted] They have not received anything She said they need a diagnosis code and a procedure code My medical insurance representative is trying to call DrS [redacted] 's office for me to get a code.I am frustrated that it is taking over months to get a simple procedure code so I can file a claim

March 25, To Whom It May Concern: Ms*** contacted my office on November 18, requesting an appointment for the removal of two temporary orthodontic anchorage devices (TADs) that were placed in The consent form my office utilizes for the placement of TADs does not state or imply
that the patient will have no financial responsibility for the removal of the TADsDue to Ms***’s specific condition and orthodontic treatment, two of her TADs had to be removed and replaced on two separate occasions and while these procedures were necessary through no fault of my own, I did not charge her for these surgeriesIn addition, Ms*** was advised prior to scheduling, it would be in her best interest to have the surgeon that is performing her corrective jaw surgery remove the TADs, but she was adamant she wanted return to my office for the surgeryWhen my staff scheduled Ms***’s surgical appointment, she was advised of the fees for the procedure and was made aware we collect payment in full on the day of surgeryShe agreed to proceed and scheduled her surgical appointmentOn the day of her surgical procedure (December 2, 2015), she was again advised of her financial responsibility and made payment without issueIn addition, Ms*** has been advised by my staff on several occasions that we are not contracted with either of her insurance carriersMy “billing person” has discussed this information with both Ms*** and her husband(TADs are part of an orthodontic treatment planTherefore, they are considered dental in nature by insurance carriers.) As a courtesy, my staff has mailed claims to her dental and medical insurance carriers as well as provided a claim form to her husband in order for Ms*** to submit a claim as wellSincerely, Richard ES*** *** *** ***
