To whom it may concern:Mr. Klepper was first seen on 10/3/14 for consultation, diagnosis, and treatment of his symptoms of TMD. At that time a pre-authorization request was submitted to his insurance carrier to pursue o maxilla-mandibular orthopedic appliance, or mouthguard. Once...

pre-authorization was obtained. Mr. Klepper was scheduled on 10/10/14 for maxillary and mandibular impressions and bite registration for analysis to design the appliance. 10/10/14 was a Friday. These cases are not routinely seen in our office on Fridays, but was done so for Mr. Klepper of his request and convenience. The case was then forwarded to the lab. The lab pick-up date is on Wednesday. which would have been 10/15/14. Once the appliance was received from the lab, Mr. Klepper was scheduled intermediately. He was seen on 11/12/14 in the Reston office which was d Wednesday. I am not in the Reston office or see patients of the Reston office on Wednesdays. Therefore I made a special trip to the Reston office on Wednesday afternoon at 3:00 p.m. to accommodate Mr. Klepper and Expedite his Case due to the laboratory delay. This time table is not consistent with Mr. Klepper's statement that it took four weeks to get the appliance from the lab and an additional two weeks before being scheduled. On 11/12/14, unfortunately offer a period of time I was unable to adjust the fabricated laboratory appliance to my satisfaction. From time to time the laboratory fabricated appliances do not meet my specifications and ore returned to the lob to be refabricated. It is not unusual when a case is returned to us that the lab can break a portion of the models during fabrication. The models taken of our office were in fact and Unaltered when forwarded to the lab, not as suggested by Mr. Kleppes. Therefore a new maxillary impression was taken on 11/12/14 to return to the lab.I advised Mr. Klepper to save time I would try to retrofit the fabricated appliance to the new model to prevent only further laboratory delay. Since was unable to accomplish this task and rather than deliver a substandard appliance, the case was returned to the lab for refabrication to my standard of care. Once again, the laboratory pick-up date is on a Wednesday which was 11/19/14, While the case was at the lab Mr. Klepper called on 12/1/4 and said he would no longer seek further treatment in our office. 12/3/14 was the anticipated delivery time from the lob, which would have been d two week turnaround time for the lob, toking into account the Thanksgiving holidays, Our office did not discontinue treatment for Mr. Klepper, that was by his choice, I hope he has found treatment for his medical condition.Unfortunately, sometimes circumstances prevent us from complying with the demands of certain patients,Mr. Klepper did not have any copays or out-of-pocket expenses. Hisaccount currently has a zero balance. The appliance and lab fees were never submitted to the insurance carrier for our reimbursements.
