RE: Customer Patient: [redacted] - Your ID #: [redacted] Dear Revdex.com:This email is in response to your attached letter we received on February 27, 2017, dated February 22, 2017, regarding ID #: [redacted] for Patient: [redacted] ***Please be advised that [redacted] received the DME: Shoulder Sling, DVT Unit (Rental) and shoulder wrap on 1/18/Attached is the Delivery Confirmation & Agreement that was signed by her husband [redacted] ***, which was delivered one (1) day before her surgery [redacted] ***'s surgery was scheduled on 1/19/2017.Our Technician ***, who delivered the DME instructed and explained to her husband: [redacted] how to use the DVT UnitAs [redacted] was not homeHowever, Monday, 1/23/2017, we received a call from [redacted] that she needed assistance in how to use the DVT UnitUnfortunately, her husband did not recall the instructions that were given to him on 1/18/On 1/23/2017, our Technician [redacted] returned [redacted] ***'s phone call, he tried reaching her times but was not able to get a hold of herFinally, later in the evening on 1/23/ [redacted] was able to reach [redacted] and she informed [redacted] that she was still in the hospital (ER) and will possibly be released later that eveningHowever, [redacted] informed [redacted] that he would call her the next morning to provide her with instructions of how to use the DVT UnitOn Tuesday, 1/24/2017, [redacted] called [redacted] to follow up with her, but [redacted] did not answer her cellular phone (562) 322-it when directly to her voicemailHe left her a message to call him back.Therefore, on 1/24/2017, [redacted] ***, the owner of Orange Coast Medical Services, Inc., spoke with [redacted] and informed [redacted] that the DVT Unit was missing the Distilled Bottle of Water for the DVT Unit when, in fact, the Distilled Bottle of Water was in the Unit.Lastly, our Technician [redacted] picked up the DVT Unit on 2/22/2017, attached is the Delivery Confirmation & Agreement [redacted] had the DVT unit from 1/18/to 2/22/2017, which is daysRevdex.com Page March 6, 2017Please be advised that we did everything possible to accommodate the Patient, [redacted] ***, as we believe in providing the best customer service to Patients.Please feel free to contact me at your Convenience if you have any questions at (562) 413-cellular or office (714) 892-7205.Respectfully, [redacted] ***
