My name is *** *** ***, I selected Orange Physical therapy because they are one of the few places that treat children under two years of ageI first called the office on Dec **,to schedule an appointment for my son for the new yearI asked the young man if there was anything that I need before the appointment and he told me noSo my appointment was schedule for Jan *, I was contacted on Jan * to the confirm the appointment by a Chelsea D***Who I asked if I needed to bring anything else to the office, she replied that I only need to bring my son's insurance cardOn Jan *, at 11:30, Chelsea D*** contacted me againShe stated that she needed my son insurance number, I gave her the number and ask if I needed anything elseShe said noMy fiance and I drove an hour and half to the office for the appointmentWe get to the office to the office at pm and Chelsea D*** says we need a referral from the DoctorMy Fiance ask if she could call the Doctor to get one, was told noBecause they could call the doctor's office and get one faxedWe proceed to the back to change my son's diaper, then after thirty minutes I went back to the front deskI was told by Chelsea D***, that the doctor's office was called and the referral was being faxedWe waited another forty-five minutes, during which we met Mrs.Cheryl G*** and MrNick M***Who seem to be the only two professionals in the officeThey were trying to mediate the situation, seem to have interest in our problemHowever the couldn't change the situation because they were only physical therapy assistants and not the business managerSo at 5:pm, the business manager comes in to speak to us, MrsAnnette F* She proceeds to go directly to my fiancee and say that the didn't get a note from the doctor's office because it was closedWe would have to reschedule to make another appointmentFirst of all you don't need a Doctor's note to see a PT in New York state, secondly her office takes paymentsI wasn't even offered if I liked to payIf I drove an hour and half to get my son treatment, I think I would pay for his screeningThe other point that bothered me was she talked to my girlfriend in a condescending mannerShe completely left me out of the conversation, even though I made the appointment and was speaking to ChelseaI wonder why, maybe because my fiancee was *** and I'm ***Did she feel I was intellectual not able to handle the situationMy whole experience was ridiculous

I first called the office on Dec **,2015 to schedule an appointment for my son for the new year. I asked the young man if there was anything that I need before the appointment and he told me no. So my appointment was schedule for Jan *, 2016. I was contacted on Jan * to the confirm the appointment by a Chelsea D[redacted]. Who I asked if I needed to bring anything else to the office, she replied that I only need to bring my son's insurance card. On Jan *, 2016 at 11:30, Chelsea D[redacted] contacted me again. She stated that she needed my son insurance number, I gave her the number and ask if I needed anything else. She said no. My fiance and I drove an hour and half to the office for the appointment. We get to the office to the office at 405 pm and Chelsea D[redacted] says we need a referral from the Doctor. My Fiance ask if she could call the Doctor to get one, was told no. Because they could call the doctor's office and get one faxed. We proceed to the back to change my son's diaper, then after thirty minutes I went back to the front desk. I was told by Chelsea D[redacted], that the doctor's office was called and the referral was being faxed.
