*** *** came into our facility on 10/29/at 5pm After the class she decided to do a membership and post dated it to 11/6/ A post date is when someone wants to join the facility, but for what ever reason they are not paying that day So their is an agreed upon
date on when the membership will be active and the payment will be ran The membership form is complete, gone over and signed while they are there in order to post date it In this case, she told the front desk staff to activate the membership on 11/6/2014. The membership became active on 11/6/as agreed *** called on 11/11/saying she wanted to cancel her membership At this time she did not say anything about being injured She was told by the Staff member working that day (***) she needs to come in and fill out a cancel form She called back later on the 11th at 2:and said she wants to cancel the membership again. All memberships have a day notice of cancellation on them This is gone over at the time that they fill out a membership form and they initial next to the cancel policy The second time she called the front desk (*** the employee) reminded *** of the day notice of cancellation This is when she said she hurt herself and could not workout She said she can bring in a note from her doctor stating she can not workout She was told by the staff she could not get a refund on the initial month, but the note can help with the day notice. To date I still have not received a doctors note to waive the billing on 12/6/2014. *** membership has already been canceled in the system Since her payment decline on 12/6/she has been called multiple times by the front desk manager about her account Calls were made on 12/11/2014, 12/17/2014, 12/30/in attempt to talk about the account and discuss the doctors note not yet received All Three times she did not answer and a message was left to call us back Since her December payment is over days late, we have already written it off. As far as not receiving a Bag, cup, or towel We do not give out a cup nor do we have cups in the facility I can only assume she is talking about the water bottle When you join you receive a bag, towel and heart rate strap Those items are received once payment is collected In her case she wanted her membership to start days after she took her initial class, so this is why she has not received them In terms of the a refund *** took her first class on 10/29/and wanted her membership to be active starting 11/6/ Knowing she postdated her membership for 11/6/she could of called at any time to retract her membership before it went active on the agreed upon date and she would have never been charged Also, once the membership went active on the 6th she has a day window where she can cancel her membership for any reason and get a full refund *** had days total to contact us about the injury We heard nothing in that time frame
