merely be a confirmation since there are no estimated coverage amounts on it at all. On 7/24/14 I contacted [redacted] at [redacted] She informed me [redacted] provided the estimate on 4/23/14. It clearly states that the plan would pay only $528.20 of the $1365 total bill. I now have a copy of the estimate in my possession. Orland Endodontics continued to confirm the remaining balance would be only $164.50, once by [redacted] when she called to confirm the appointment and again in writing on the day of the procedure. [redacted] at [redacted] assured me that they only submitted one estimate. I believe that it was the responsibility of Orland Endodontics to accurately inform my wife, [redacted], of the estimate that was provided by [redacted] Therefore I believe I should not be held responsible for the charges above and beyond the proper insurance estimate.

We received further correspondence from the "consumer" today, August 4, 2014. Again, we would like to make it clear that Mr. [redacted] is NOT a consumer of our office. He is not a patient of record, nor has he ever physically been in the office.
Through our initial response, we clearly indicated that verification of a patient's insurance benefits is only an estimate. On the SIGNED financial agreement that was already provided, it is clearly stated that estimation and filing of insurance benefits is completed as a courtesy to patients. There is no law that states estimates must be provided by a medical provider.
Mr. [redacted] has clearly demonstrated that he has the ability to contact his own insurance carrier. If the out of network fees were concerning to him, he had every opportunity to call and verify benefits himself before services were rendered.
Mr. [redacted] has made no effort to pay anything on the outstanding balance. He was given written notice that the account would be turned over to collections if no payment was made. As of August 1, 2014, the entire account has been turned over to a collection agency.
Orland Endodontics will make no further communications regarding this account.
