One Ortho Northeast must honor its promise and bill my Indiana Hoosier Health Care Insurance number #XXXXXXXXXXXXX for medical payment on 10/28/13.
On Wednesday, October 16, 2013 at about 3:39PM I received a telephone call from One-Ortho NorthEast, informing me that they needed my Health Insurance Card number, prior to my doctor's office visit scheduled on October 28, 2013, at 2:45PM with Dr. [redacted] After I provided my Medicaid Hoosier Health Care Insurance number. I then received an online E-mail application to complete. The Customer Representative told me to bring my health insurance card to the doctor visit, because they would need to scan the card into Its system. If I did not have any insurance, immediate payment would be required at the initial check-in.

I complied and on October 28, 2013, when I arrived for my doctor appointment I checked in at the front desk and was guided to another station where I provided my Insurance card and identification information that was scanned into Ortho NorthEast system.

After the doctor visit I was then guided to check-out, the Registrar told me Ortho NorthEast would bill my Medicaid Hoosier Health Care, handed me a Clinical Visit Summary and told me I could leave and have a nice day.

On Friday, November 8, 2013 I received an "Anonymous" telephone call from One-Ortho NorthEast Customer Representative informing me that I would receive a bill in the mail for the office visit because Ortho DID NOT bill Medicaid and while I was asking her a question she abruptly hung up the phone. Within minutes I quickly called Ortho North East billing department to have them explain to me why they refused to bill my insurance. Each of the 4 times, that I continued to call I was placed on hold then the call was disconnected.

On November 13, 2013 I received a bill in the amount of [redacted] from One Ortho Northeast. I need One Ortho NorthEast to comply in billing my Medicaid Hoosier Health Care Insurance as agreed on Wednesday, October 16, 2013 and again on October 28, 2013 for the full $399.00. I did NOT refer myself to Dr. [redacted] my Primary doctor's nurse made the referral arrangements, I did not know of Dr. Hoffman prior to the visit.
