original payment to OFA was taken back because the orthotics were determined to be a non-covered service upon review by [redacted]. [redacted] advised the patient that she could appeal it with [redacted] since they were originally told by [redacted] that it would be covered. This is an issue of payment responsibility between [redacted] and the patient – OFAC charged $300 for the orthotics and has not been paid by either party at this point. Please refer to page 33-34 of the attached [redacted] Orthotics Policy. Our orthotic code is L3000 and L3020 which the policy clearly states is “NOT COVERED.”
