The policy regarding the completion of forms at Orthopedic Specialties is:
Completion of forms may take up 7-business days to processThe fee is $per form and must be paid when the form is received by the officeForms received and paid for on the same day are discounted $for the
first form and $for each additional form. If additional forms are received and paid prior to completion of the first form we will honor the $fee per additional form
Multiple forms take less time when completed at once while forms completed on multiple days consume more time. When a request is made to mail a form as soon as possible we try to honor that request to the best of our abilityIf the patient knows a second form is coming and would like for us to hold the original form we would be happy to do soWe have no control over when forms are received or the time of day they are receivedIf additional forms are received and paid outside of the same day policy and the original form has been completed and mailed/faxed already we charge $for the new form as per policy
