Review: I received medical services on May 21, 2014. I had gone to a new doctor for a well visit/annual physical. The lab work I had done was not coded as a well visit/annual physical and my insurance company had not been updated that I have no other insurance so I received a bill, statement date 7/3/14 for $1,011.54. I updated with my insurance company that I had no other insurance and called Our Lady of the Lake (OLL) RMC on 7/11 and was a told to disregard this bill and wait for new statement. I also called the doctors office 7/14, 7/15, and was told on 7/16/14 that they had refiled the bill with the correct codes to OLL RMC. Received statement dated 8/3/14 still being billed $579.51 (my insurance pays 100% for well visit/annual physical). Called doctor's office 8/12 and confirmed new codes were sent to OLL RMC. Called OLL RMC 8/12 (Jacquita) and was told new codes were in the system and would be refiled. Statement 9/3 no change, called 9/9 ([redacted]) told it would be taken care of today and refiled in next few day. Called again 9/23 ([redacted]) still not done, transferred to Francis (supervisor) and told would be done. Statement 10/3 no change 10/14 [redacted] then [redacted]) would be done. 10/17 received call from [redacted] asking to set up payment plan for overdue bill??!!! 10/27 ([redacted]) told was submitted 10/21 to insurance company. Statement 11/3 no change so called 11/11 OLL RMC, [redacted] told me to call business office, account transferred there, called them ([redacted] then supervisor [redacted] told it was refiled 10/30/14. My insurance company, UMR, has received nothing as of 11/11/14; told this to [redacted] and she said she would notify Insurance Dept. that nothing received by UMR. Gave them my insurance (UMR) fax number to maybe expedite claim. Also talked to Francis (OLL RMC) on 11/11/14 who was also looking into it. So as of today, 4 months after new codes sent in, problem has not been corrected. Our Lady of the Lake RMC PO Box 61008 New Orleans ###-###-####Desired Settlement: Would like the Revdex.com to investigate this issue as maybe they can resolve this issue whereas I have not been able to. Feel it should not have taken this long to refile the claim. Was told I could not talk to Insurance Department directly.

Review: On January 21, 2013, I visited my cardiologist for my yearly check up. My cardiologist sent me downstairs to get lab work. I was unaware that OLOL had taken over the lab and that my insurance would not cover the lab work. I received a bill from OLOL for $200.00 for the lab work in June of 2013, five months after the date of service. I spoke with many different people at OLOL stating that I shouldn't be responsible for the charges and if the insurance did not cover it OLOL would at the very least reduce the charges. I was told they would investigate and get back with me. Finally, after receiving a statement every month (and calling to dispute every month), I was informed that the supervisor, Matt, had approved a 15% discount. I spoke with [redacted] and he assured me that if I paid the discounted amount that it would be paid in full. Even though I thought this discount was not a good one, I was ready to make this mess go away. On October 12, 2013, I mailed a check to OLOL for the "discounted" amount and that check has since cleared my bank. I continue to receive monthly statements showing a balance and I have continued to call OLOL every month only to be told it has been taken care of.Desired Settlement: I would like to stop receiving the statements since my account has long been paid in full.

Review: I went to Dr.[redacted] for sinus trouble on the first visit he diagnosed that I needed surgery. I signed paper work for out-patient surgery to be performed. The next time I saw him I paid $200.00 copay and had the surgery done in the Hospital. He scheduled a follow up within one week to check on his work and I returned for the apt. After seeing him he scheduled another apt follow up with in two weeks which I attened. I then recieved bills for outpatient surgery. Every time I saw him in his office he charge me for out patient surgery with out my consent. He and the hospital are charging me with $600.00 of out patient deductable surgery fees. This is a very deceptive billing practice and when I called him about it he said he is on a saalary and has no control over it. many people have been fooled into this and its time for this to stop taking advantage of people.Desired Settlement: Remove these charges from my account.
