I am in receipt of the recent request to a complaint against the Immediate Care Center, located in Crown Point, IN.
However, given...

the nature of this complaint and if regarding a patient visit I cannot release any information in reference to a visit without a properly executed signed consent from a patient.
Additionally, I would need to have all the appropriate demographic information concerning a patient as to make sure it is for that patient and not another. This would have to include, patient name, date of birth, date of service, and social security number.
