On Thursday 08·28·14 patient [redacted] was In our office, after Doctors examination It was determined patient needed 3 crown.
Patient Informed that medical will not pay for the crowns and that he had option of [redacted] payment plan. Patient...

agreed to complete application online needed help,one of our staff 
members help him complete lt. [redacted] was approved bv [redacted] and decided to start with treatment.Patient signed treatment consent and Dr. proceeded with the treatment of doing the 3 
crowns.
After treatment was done patient was dismissed from operatory, once at the front desk he did not  sales draft for the amt. of$ 1875.00. Once more we went over promotional payment plan,at that time
[redacted] told  us he did not think he could make $105.00 payments.

Patient was given option payment plans,he decided for the 48 month,$ 53.00 per mo. Patient was explained that original sales transaction needed to be voided and a new transaction would have to 
be done for his pmt to be $53.00 Pet month,also it was explained to patient that once voided original transaction we would have to wait 48 hours for funds to become available to plate new transaction
.Patient agreed and since he needed to come back In 2 weeks for delivery of permanent crowns, he agreed to sign new sales slip 
on 09 15·14.
At no time patient was under the impression that he did not have to pay for services. 
Documentation to supporting this matter Is on file.

Review: 8/16/13 - The dentist office charged my credit card for work already done on 8/14/2013. 5/7/2014 - I was told by the clerk that since I had a credit to my account in case the insurance does not cover, but they will because Arestin is cover by your insurance, that they will use that money toward the services.I asked why I was not told about it? she said I did not know until just now.I went home thinking about it, I called the insurance to verify if Arestin is a cover services by them and the answear was yes. 5/11/14 - I called the office and I asked them to reimburse my credit to me. She indicated the office is in LA and it will take a month for me to get the money. I said OK. I will wait a month.6/14/14 - I went to the dental office and I asked to speak to the manager. She was aware of the double charges and at that time she ask the clerk to follow-up by calling me. I never received a call or message. I also indicated that I received the EOB from the insurance in which they dennied the Arestin, they billed for 8 Arestin, not 7 (even making the correction to the clerk), they billed for "unspecified item" which for sure the insurance dennied. She checked the records if clerk called the LA office, and yes she did (on paper).Manager apologized, she promised me she will take care of the matter and that she will re-submit information to the insurance.Today is 2 weeks since that last conversation. I truly don't think it is correct to keep that money ($300+)for 11 months. For sure I will change dentist. Makes me wonder if he is aware of the practice in his office. It will be great if you can help me to speed this matter. Thank you. Desired Settlement: I just want my money back and appropiate billing to the insurance.

Review: I have an issue that I have been trying to handle with this dental office and it has taken a long time to resolve. I was seen by them in 07/2014 and again in 09/2014. I was charged upfront both times and a few weeks later I find out they over charged me and never contacted me to let me know I had a credit owed to me. I personally showed the manager the Explanation of Benefits that I had received for the service in 07/2014 and I asked if she could help me understand why my insurance stated I was only responsible for $15.00 when I had paid $295.00. She gave me an explanation that made sense to me at the time, considering this had been my dental office for more than 10 years. I trusted her explanation. Since that conversation she agreed that the only credit I had was $70.00. So then I received my Explanation of Benefits for the second service that took place in 09/2014. This showed my responsibility to the dental office was ZERO amount when I had been charged $365. That is when I felt really confused. So I decided to call my insurance company. They explained to me that they paid the dental office in full according to the contract that is between the dental office and the insurance. They specified I only had to pay $15.00 total for both visits. They also explained that the way the dental office was billing was incorrect. They were trying to bill the same code more than once for the same tooth. Per the contact they can not do that. I informed the manager right away. Since then, it was run around answers from the manager. One of her last answers was that she submitted the paperwork to the accounting department that handles the credits. I come to find out from one of the receptionist that the manager handles all of the accounting. Something doesn't seem right. There are too many answers I had been getting that do not make sense. I called on 11/10/14 to ask for contact info of someone above the manager. The receptionist did not provide any contact person. The next day I received a portion of it. Not allDesired Settlement: I did not receive the full amount of my credit. I hope to have that resolved soon. My biggest hope in this matter is to put a stop to this unethical billing practice. I truly feel I am not the first victim. I might just be one of the first that has caught on to their wrong doing. My trust of more than 10 years in this dental office is gone. I will never go back.
