dentist to our specialty Endodontic practice on 8/18/2014 for evaluation and treatment of the upper right quadrant. Root canal treatment had been previously initiated on tooth #3 at a previous dental office. After informed consent was attained, root canal treatment was completed at our office on tooth #3 on 10/20/2014. Documentation that root canal treatment was completed on tooth #3 includes radiographs and the final report . We would be more than happy to provide these dental records to verify completion of endodontic treatment on tooth #3. On 6/8/15 I was able to contact patient's mother in order to clarify the situation. His mother said that patient is still having discomfort on the upper right side including hot/cold sensitivity. I informed her that another tooth may be involved and recommend to have patient come back to our office for reevaluation free of charge. If the patient schedules a follow-up, we can see what are his symptoms and determine the correct treatment protocol in order to resolve this matter.
