To Revdex.com:
This is the history of Mrs. [redacted] appointments with us here at Pacific Family Dentistry.2/24/15- Mrs. [redacted] and her husband came to visit Dr. S[redacted] at Pacific Family Dentistryfor an evaluation of her lower right molar tooth, #30. Mrs. [redacted] was upset and...

expresseddissatisfaction of her dental treatment from her previous dental clinic. She stated that hertooth #30 was treated with a root canal therapy by her previous dentist and her old crown on#30 (about 40 years old, according to her statement) kept falling out, and wanted a new crown. Her chief complaint was documented in her dental record.
3/5/15- Mrs. [redacted] visited us for a new crown preparation on tooth #30. The tooth wassensitive to percussion on that day and Dr. S[redacted] suggested that she should consider returning toher previous dentist or any root canal specialist for any possible root fracture or a need forretreatment before a new crown can be fabricate. Mrs. [redacted] declined Dr. S[redacted]'s suggestionto get a second opinion by stating that she had been to different dentist(s) before and was toldthat the tooth was fine and wanted to get a new crown now.
She stated that if the tooth hurts in the future, she will just extract the tooth and get animplant. Dr. S[redacted] reiterated that if the tooth becomes symptomatic, especially during her twoweeks of temporary crown, she should visit a specialist before a permanent crown can becemented. This conversation was also documented in her dental record.

3/23/15 - Mrs. [redacted] returned for a crown cementation. The crown fitted well and verifiedwith an X-ray and her bite and comfort was checked. She was instructed to return forrecall/cleaning appointment.
11/24/15 (8 months after last apt.)- Mrs. [redacted] called and spoke with one of my staffs,Bethany. Mrs. [redacted] complained of sensitivity from cold and hot temperature, and shethought it was coming from a tooth #30. She demanded a free exam but we refused. My staff,Bethany, told her that there would be a fee for an exam and any necessary x-ray(s). Mrs.[redacted] became aggressive and argumentative and was not willing to hear Bethany'sexplanation. After close to 50 minutes of this conversation, it was time to end theconversation.
Her husband, Mr. [redacted] called back, and Bethany informed him that it would be the best tosee a different provider and gave him a referral.
Comment1. We don't know the source of her sensitivity/pain (it could be many different things)unless we examine her and it won't be free of charge. Usually any symptom related totemperature like her case is related to her nerve, not the crown itself. We won't knowfor sure until the exam. She has not even returned for a recommended regular checkupand cleaning (it is her responsibility) since her last appointment. Now she refuses topay for an exam/x-ray fee to pinpoint the cause of the discomfort and wants a fullrefund? I don't think so. It is not logical and does not make sense.
2. She automatically assumes and points finger to us that we are responsible without aprofessional examination/diagnosis. Now I learned that she also filed a complaint withRevdex.com regarding her previous dentist to get a refund as well. Is there a pattern here?
