This patient was seen at our sister office in Oroville on November 11, during a special charity eventHe choose to call our Paradise office to schedule his next appointment in May of He was informed they are separate offices and because the businesses are not connected in any way we
would need to do an exam to establish and become a patient of recordHe understood and agreedHis records and old x-rays were transferred from the other office before his visitHis insurance was verified and coverage was entered based on the information his insurance provided He was seen by Dr *** ***, an associate Dentist at the officeBite-wing x-rays were taken as it is standard practice for those to be taken annually and the existing records were a year and a half oldHis insurance was billed and payment was receivedThe amounts covered were different from the information his insurance had provided us as they pay off their own fee schedule of which we were unawareThe patient came into the office very upset months laterHe initially refused to give his name to bring up his account for us to review it with him and answer questionsHe was very abrasive and angryHe threatened to leave bad reviews and repeatedly called us names Toward the end of his time in the office he provided his billing statement but was still not open to a conversationNo progress was madeIt was offered to have Dr *** call him and he gave an ambiguous answerHe left without allowing a conversation to happen or requesting anything specificA letter to Dr*** has not been receivedNor has there been any other communication on from the patient

This is the text from document attached. Thank you,  Mr. [redacted],Your dissatisfaction with our efforts to take care of your dental needs has been relayed to me along with the correspondence we have received directly from you and through the Revdex.com. After reviewing all these it appears your main concern is the $118.35 balance on your account at Paradise Gentle Dentistry. If it would be satisfactory to you, we will write off the entire balance and add it to the $580 in free dentistry you have received from my offices. I regret there has been so much confusion regarding your billing.According to our documentation you were first seen in the Oroville location on November 11, 2014 by Dr. [redacted] who did Panoramic and Bite Wing radiographs, a Velscope® oral cancer screen, and a comprehensive exam as part of our annual free day of dentistry for veterans (Thank you for your service). One of the hygienists also did a prophylaxis and applied fluoride to your teeth.The next time you were seen was the following year, November 11, 2015, when I saw you myself, updated your Bite Wing radiographs, did a complete exam, and you saw the hygienist for another prophylaxis and fluoride application.When you were seen in the Paradise location on May 6, 2016 by Dr. [redacted], your radiographs were updated (which would be typical for someone being seen as a new patient not having current, within the last six months, radiographs) by taking four Bite Wing radiographs, (we did transfer the previous radiographs including the panoramic)  he also did a new Velscope® oral cancer screening and complete exam. They hygienist provided a prophylaxis for you but did not apply the fluoride varnish. I’m assuming that was in an effort to be sensitive to your costs.Typically the charge for that visit would be $270, we immediately wrote off, or discounted, $35 for the Velscope® oral cancer screening. In accordance with the rules of your insurance company we submitted all the procedures to them for consideration for payment. Your insurance company pays percentages according to their own fee schedule which we are not able to see. They paid $116.65 of the $270 charge. We had, and still have, no way of knowing how much they would pay prior to receiving their payment. Their representative had informed our team member that they paid all of the costs for preventative care but failed to mention it was “all of the costs” according to their fee schedule.As we try to be honest and ethical with all our dealings with patients and insurance companies we are also bound by the insurance companies rules, even when they don’t give us complete information, which is why you ended up with a balance contrary to what we informed you based on the information we had received from them.Hopefully this explanation will help you understand our efforts to be charitable and honest with you as well as deal honestly with your insurance company.Our efforts to thank veterans by offering the free day of dentistry has caused some confusion since the service is provided by team members from both offices even though they are truly separate entities. Even more unfortunately, your insurance rules and restrictions made it even more confusing. We will still continue to offer this gift in gratitude and respect to other veterans like you.As stated above, we will gladly write off the remaining balance on your account and happily, at no charge, forward your radiographs to the dentist of your choosing. I’ve attached acknowledgment and release of records forms to this letter. As soon as we receive them we will remove the balance, take you off all our correspondence lists, and forward your records to the dentist of your choice. We will be available to attend to your emergency dental needs for the next 30 days. This should give you ample opportunity to select another dentist.
