We received Ms***’ original distribution request on June 2, The claim was processed and the check was mailed out by our trustee *** *** on July to Ms***’ address of recordMs*** informed us on September that she had not
received the checkWe then placed a stop payment on the original check, and a second check was mailed to her address by *** *** on September We were having phone service troubles with our provider *** on October 6, which is the day Ms*** states that she tried calling our office and received the “number disconnected” message
We received the *** complaint on Oct and called Ms*** *hat same dayWe explained our phone issues from October and described what the check would look like: a pressure sealed envelope from *** *** that does not say *** on itUpon our describing the envelope, Ms*** paused for several seconds, as if she remembered seeing the envelope and had possibly thrown it awayThis is conjecture, but we hear this very frequently from other participants (“I thought it was junk mail”), and this is usually the reason that we have to ask *** *** to reissue distribution checksMs*** *hen said she would review her mail when she got home to see if she had received the check
Having not heard back from Ms*** in over a week, today we checked the *** *** website and saw that the check had not been cashedWe called her today to follow up and she said that she has not received the second checkWe informed her that we will be placing a stop payment on the second check, that we will request a third check from *** ***, and we will be sending it to her address via *** *** *** *** when it arrives next week
