Response:The x-rays which were not covered were for the patient's first visit to our office in 2014. As with all new patients we do require a full set of x-rays unless one is provided to us by the patient. Our insurance verification process for that visit did indicate that a full set is payable once...

every 5 years and no history of x-rays was provided to us by the carrier. Once the claim was processed it was denied for frequency. Our follow up of that denial provided information that the patient had this x-ray in 2012, again information which was not provided to us during our insurance verification. The insurance maximum allowable for all x-rays taken in 2014 was $79 which was billed to the patient. An $11.00 collection fee has since been added once the account was submitted to collections. We do have copies of the processed EOBs as should the subscriber showing the denial for the 2014 visit.
