Revdex.com: I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below My rebuttal to the response submitted by Ohio Spine and Body regarding the Revdex.com complaint I filed on 10/22: 1) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: While we do agree with the fact that her treatment plan was not completed as scheduled, the details of the situation as described by MrsMarshal are in fact, not accurate [redacted] 's response: Please see #through #for a detailed accurate rebuttal to the response of the complaint provided by Ohio Spine and Body 2) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: We rescheduled the patients who were being seen by this physician to the day we expected him to resume his usual scheduleThe physician then did not return to the clinic and did not communicate as to his reason for not returning or even notifying us in advance of his intentions to not return [redacted] 's response: This statement by Ohio Spine and Body is false: Dr [redacted] had explained that Dr [redacted] quit because of a family emergencyHe said he was not authorized to write a prescription for phentamine and was in the process of trying to find a physician, but it was hard finding a physician I had an appointment on 10/at 10a.m [redacted] (stated during our conversation that she was a new employee of only weeks) called me on 10/or 10/(can't remember which day) and told me that I had to reschedule because Dr [redacted] had a family emergency and would not be in I explained to her I would be out of the phentamine on Monday, 10/and needed to come in to get the remaining two weeks refilled Originally, I set up my appointment to see Dr [redacted] on 10/at a.mso I could let him know how the program was going and discuss any concerns --I didn't realize at the time he was the only one that could write the prescription until I met with Dr [redacted] that day— [redacted] told me on the phone to come in to get the phentamine When I arrived at a.mon 10/13, " [redacted] " a staff employee (not sure of her title) took me in the backroom and asked me if I needed a lipo injection I told her "no" I had been given the lipo syringes on 10/to be used at home I explained to her that I was there for the phentamine She said "I knew it, wait here while I get Dr***." This is when Dr [redacted] came in and told me that he was not authorized to write the phentamine, to wait a week and come back on Friday, 10/because he was trying to get another physician to take over the weight loss program Dr [redacted] told me he would add an extra week at the end of my program since I wasn't able to continue without the phentamine from Monday 10/to Monday 10/20, of which I responded very politely, ok I understand how emergencies go Please make sure you write down in my file that you will be adding a week to my weight loss program This was a calm conversation Dr [redacted] said, during the week without the phentamine, “eat green vegetables, stay off milk and cheese, and you should at least maintain the weight that you lost.” Please note: When I had originally called to inquire and enroll in the program--it was around 9/10/14--I was told by [redacted] [redacted] , Case Manager, who handles the weight loss program for Ohio Spine and Body, that Dr [redacted] was out on a family emergency and Dr [redacted] would not be back until Monday 9/which is when I had my first appointment [redacted] met with me, she filled out the paperwork because she lost the other set I had originally filled out—during mid September when I first inquired about the program On 9/I explained that I could fill out the paperwork, [redacted] said “no that’s alright, I’ll fill out the paperwork for you.” I signed it, but was never given a copy of the paperwork nor was I given the opportunity to read it, I simply signed them I met with Dr [redacted] that dayI was told that my program would start on 9/ 3) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: Once she was informed we were not able to deliver the rest of this medication, Mrs [redacted] became combative and insisted she receive a refund immediatelyThe dollar amount she has requested has changed from $to $The original package purchase price was $ [redacted] 's response: When I came back Friday, 10/(appointment was prescheduled with Ohio Spine and Body at 10a.m.) Dr [redacted] confirmed that he wasn’t able to get a physician and wasn’t sure if he would have one by 10/ I told Dr [redacted] I didn’t want to continue the program since I had signed up for a month and the stopping and uncertainty of hiring a physician was hindering me from losing the weight Dr [redacted] said he would call me Friday evening, 10/to let me know how much I would be reimbursed He never called me I had also asked him, “Do I need to make an appointment for Monday, 10/20? He said no just come in and I can write you a check I told him, I paid by credit card, Dr [redacted] looked through my file and said that’s right I thought you paid by check, I can give a credit to your card on Monday, but you would have to come in since I don’t have your credit card number I said ok and left It was a calm conversation the whole time Dr [redacted] never mentioned a day policy refund When I came back on Monday, 10/at 12:15, (Please note: Dr [redacted] stated I didn’t need an appointment when I asked him on Friday, 10/17) I was told by the receptionist that Dr [redacted] wasn’t there I told the receptionist I would wait, and I waited about minutes I got up and asked the receptionist if she knew how much longer it would be before Dr [redacted] would be back She told me “I’m not sure,” “let me call him again.” The receptionist called Dr [redacted] and he told her he would be right there About minutes later, Dr [redacted] came through the door, and I said hi Dr*** He said Hi and walked past me About minutes later, Dr [redacted] called me into the side office He explained that they weren’t able to find a physician and he made a mistake--I would have to wait days for my refund I responded calmly and stated, you told me you were going to call me on Friday, 10/to give me the amount due and you never called I told him you had asked me to come in to get the refund today (Monday 10/20) I told him I’m not waiting days to get my refund since you told me it would be refunded today I asked him to call HR to inquire how much they were going to refund me, and I would be happy to wait while he called them This was a calm discussion—no yelling occurred Dr [redacted] said ok he got up and went in the back room to call HR I waited in the lobby for about ½ hour and finally I got up and asked the receptionist if Dr [redacted] was still on the phone with HR The receptionist responded “yes” Approximately minutes later, Dr [redacted] came out of the back room and took another patient into the room He never said anything to me I waited about minutes, went up to the receptionist, and asked her why did Dr [redacted] had taken another patient since I had been waiting for him to get off the phone with HR, and he never said anything to me about what transpired with HR? The receptionist said she would ask Dr*** So the receptionist went and knocked on the door where Dr [redacted] was with the patient, and they both went into the back room area to discuss The next think I knew, [redacted] (I did not know her name until 10/21—see #below) came and asked me to come in the back room with her—she shut the door She explained she was with HR and was responsible for billing (This seemed strange to me since I was told Dr [redacted] was on the phone speaking to HR???) [redacted] started to tell me that Dr [redacted] didn’t quit I interrupted her and told her yes he did, because Dr [redacted] told me Dr [redacted] had quit When I went to explain what had previously happened she started talking when I was talking I told her to wait until I finished, but she kept talkingI did get loud at this time because she wouldn’t stop So I told her to shut up and listen to what I had to say I began to get angry because of the treatment received by Dr [redacted] and now her All I wanted was my credit She told me that I had been on a promotional program and that it was a low cost program already I told her I didn’t care—I didn’t cause Dr [redacted] to quit and I was there to get my money refunded I still paid for four weeks and it was plus tax—and I only was able to use two weeks She told me she would call me Tuesday, 10/to let me know how much was going to be reimbursed, and I told her don’t bother I would be in for the credit on Tuesday 10/and I left [redacted] never said I had to wait days for my refund 4) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: Mrs [redacted] returned to the office without notice three times between 10/and 10/demanding her money immediatelyThis was during normally scheduled patient times when we are assisting other patients with their treatmentWe made every attempt each time to direct her to a private room to address her concerns and assure her that her account would be handled according to our office policy [redacted] 's response: This statement by Ohio Spine and Body is false: I returned on 10/and I had a scheduled appointment was at am On Monday, 10/Dr [redacted] told me I did not need an appointment—I arrived about 12:15p, 10/21, One Wednesday, 10/22, I arrived at approximately 12:30p.msince Dr [redacted] told me to come back See details below: On Tuesday 10/21/(as I had told [redacted] on 10/I would be coming in) I went to Ohio Spine and Body to get my refund When I got there, the receptionist asked me what I wanted I explained calmly I was there to see [redacted] for my refund She started to escort me to the side room off of the reception area, but I told her I wasn’t going into any more meeting rooms I wanted my refund-calmly About minutes later Dr [redacted] came out of the back area, and he stares at me so I stared back Finally, he tells me that [redacted] wasn’t there I told him why would [redacted] tell me she was going to call me today, knew I was coming in today, and all of a sudden now you’re saying she is out??? He said I don’t know why she would tell you that I told him to call her on her cell phone and ask her how much the refund would be Dr [redacted] told me “no” he wasn’t going to call her on her cell phone I asked him why he was not refunding my money since I didn’t cause this—it was Dr [redacted] who resigned Dr [redacted] said I don’t know how much you will be getting refunded since [redacted] is handling it Since I had been in there the day before I yelled at him and told him yes you do know because you discussed it with her before and after I left I was angry at this time because I had been going back and forth to Ohio Spine and Body on 10/13, 10/17, 10/and 10/ He told me “too bad”, I wasn’t going to get my refund, he wasn’t going to call her on her cell phone, and to come back when [redacted] would be there on Wednesday, 10/to get my refund I went out to my car and waited because I was so angry I went back inside and picked up a business card and asked Dr [redacted] what was her name (I never had [redacted] ’s name until 10/21) Dr [redacted] told me her name was [redacted] , I wrote it on the business card and told Dr [redacted] that I would be back in on Wednesday 10/and he had better give me my refund or I was going to call the police I also asked him how he could live with himself being a liar and a thiefYes I did yell this at him There were no patients in the waiting area—please see office hours in # Please note: I left and went to my car, I called my husband on my cell phone—told him what happened, and asked him to call Ohio Spine and Body to make sure that [redacted] would be in on 10/22—I had been given the run around so many times—I wanted to make sure she would be in He called, and was told [redacted] was schedule to be in at a.m When my husband called me back and told me, I asked him if they told him whether [redacted] was going to be in the entire day He said, they didn't tell him I asked him to call them back to inquire her workday schedule He called them back and the receptionist told him [redacted] would be in the entire day on 10/ My husband left his name and phone number [redacted] never called him back I have the phone record to prove my husband called twice 5) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: Each interaction with Mrs [redacted] resulted in her becoming combative and raising her voice to a level other patients were disruptedThe final interaction resulted in the police being contacted and a complaint being reported by our clinic director [redacted] 's response: This statement by Ohio Spine and Body is false: Yes, who wouldn’t be combative—please see definition regarding combative (adj.) from the “Free Dictionary by Farlex” Inclined or showing an inclination to dispute or disagree even to engage in lawsuits, a style described as abrasive and contentious; a disputatious lawyer Again, please see each day/time I came into Ohio Spine and Body: Monday 10/and Friday 10/17—Met with Dr [redacted] at a.mboth days--calm conversation Monday 10/--Met with Dr [redacted] in side office—Asked him calmly to call HR to get my refund—which Dr [redacted] agreed and went in the back room to call HR (door was closed no patients in room)—later [redacted] came and asked me to come into the back room—she was rude and interrupted me and I yelled and told her to shut up I told Dr [redacted] and [redacted] I would be back on 10/ Tuesday 10/21— Came at 12:15—no patients were present (see office hours below), both [redacted] and Dr [redacted] were told I would be in Wednesday 10/22—Came to Ohio Spine and Body to get refund at 12:doors were locked—no patients were present I knocked on the door, Dr [redacted] let me in Please note: Office hours for Ohio Spine and Body: Monday: 9a.mto 11:30a.m., 2p.mto 5:30p.m Tuesday: 8a.mto 11:a.m., 2p.mto 4:30p.m Wednesday, 9a.mto 11:a.m., 3p.mto 5:30p.m Thursday, 8a.mto 11:a.m., 2p.mto 4:30p.m Friday, 9a.mto 11:a.m., 2p.mto 5:30p.m On 10/and 10/I met with Dr [redacted] and [redacted] behind closed doors When I came in on 10/and 10/what patients were disruptive since I came when no patients were scheduled (see office hours above)? The doors were open except on 10/when I told them on 10/I would be back on 10/to get my refund and if I didn’t get the refund, I would call the police Dr [redacted] had the doors locked when I arrived at 12:30p.mon 10/ I knocked on the door and stood there for about minutes Dr [redacted] came and opened the door I came inside and he asked “what do you want in a sarcastic tone.” I told him calmly I was there for my refund since he told me to come back on 10/ He said [redacted] wasn’t there (again)I told him I would wait He stated she was not going to be there all day She was in Cincinnati I told him I would wait while he called her on her cell phone in order for me to get my refund—no yelling occurred He said if you don’t leave I am going to call the police I told him go ahead because I was going to call the police too Dr [redacted] called the police I also called the police while in the lobby of Ohio Spine and Body The female dispatcher told me to leave because it was private property I explained to the dispatcher that I had been back to Ohio Spine and Body numerous times trying to get my refund, and Dr [redacted] wasn’t refunding my money She said she understood and suggested I take the matter to the small claims court, but I was to leave I immediately left and went directly to the small claims court on Wednesday, 10/to file a claim I had to pay $to file the claim 6) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: The situation was handled professionally and respectfully by our staff, as would be expected by a clinic that specializes in customer service and attention to patient care [redacted] 's response: This statement by Ohio Spine and Body is false: Seriously??? How can they make the statement professionally and respectfully?? Why was I not given my refund when Dr [redacted] told me times to come back, times I communicated with [redacted] and Dr [redacted] I was coming back the next day, and times I was told I would be called to let me know the amount of the refund? Please explain how this is considered respectful and professional treatment 7) I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: The behavior exhibited by Mrs [redacted] was erratic, combative, uncooperative and disrespectful to our patients and staffHowever, we will continue to process any refund that may be applicable in accordance with our office policy [redacted] 's response: This statement by Ohio Spine and Body is false: I am requesting $refund from Ohio Body and Spine Originally, I would have settled for $200—this is the $per week for a total of two weeks I wasn’t able to complete the program I can’t use the lipo injections if I can’t have the phentamine or a physician to discuss my weight loss concerns But since I had to make a total of trips back and forth ($for mileage each day), go to the small claims court and pay $78, I figured the extra gas, my time, mistreatment, and frustration on not getting my money refunded--Ohio Spine and Body should repay the full amount--$plus tax The bottom line is I paid for weeks of a weight loss program and because of a situation out of my control, and after coming back to Ohio Spine and Body numerous times, left each time without a refund It should never have come to me having to file a claim to the Revdex.com or go to a small claims court since a refund was confirmed by Dr [redacted] to be credited through a quick swipe of my credit card Ohio Spine and Body’s policy is to pay for the program before starting it and using my credit card resulted in a transaction in a matter of seconds Since Dr [redacted] and [redacted] stated they would credit my credit card, the transaction would have involved a matter of seconds to perform thus providing Ohio Spine and Body the opportunity to practice the “specialized” customer service and “attention” to patient care as stated above—but unfortunately they did not do so at all—very disappointing Regards, [redacted]

The next think I knew, [redacted] (I did not know her name until 10/21—see #4 below) came and asked me to come in the back room with her—she shut the door.  She explained she was with HR and was responsible for billing (This seemed strange to me since I was told Dr. [redacted] was on the phone speaking to HR???)  [redacted] started to tell me that Dr. [redacted] didn’t quit.  I interrupted her and told her yes he did, because Dr. [redacted] told me Dr. [redacted] had quit.  When I went to explain what had previously happened she started talking when I was talking.  I told her to wait until I finished, but she kept talking. I did get loud at this time because she wouldn’t stop.  So I told her to shut up and listen to what I had to say.  I began to get angry because of the treatment received by Dr. [redacted] and now her.  All I wanted was my credit.   She told me that I had been on a promotional program and that it was a low cost program already.  I told her I didn’t care—I didn’t cause Dr. [redacted] to quit and I was there to get my money refunded.  I still paid for four weeks and it was 396 plus tax—and I only was able to use two weeks.  She told me she would call me Tuesday, 10/21 to let me know how much was going to be reimbursed, and I told her don’t bother I would be in for the credit on Tuesday 10/21 and I left.  [redacted] never said I had to wait 30 days for my refund.

I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: Mrs. [redacted] returned to the office without notice three times between 10/15 and 10/21 demanding her money immediately. This was during normally scheduled patient times when we are assisting other patients with their treatment. We made every attempt each time to direct her to a private room to address her concerns and assure her that her account would be handled according to our office policy.
[redacted]'s response: This statement by Ohio Spine and Body is false:  I returned on 10/13 and I had a scheduled appointment was at 10 am.  On Monday, 10/20 Dr. [redacted] told me I did not need an appointment—I arrived about 12:15p, 10/21, One Wednesday, 10/22, I arrived at approximately 12:30p.m. since Dr. [redacted] told me to come back.  See details below:
On Tuesday 10/21/14 (as I had told [redacted] on 10/20 I would be coming in) I went to Ohio Spine and Body to get my refund.  When I got there, the receptionist asked me what I wanted.  I explained calmly I was there to see [redacted] for my refund.  She started to escort me to the side room off of the reception area, but I told her I wasn’t going into any more meeting rooms.  I wanted my refund-calmly.  About 4 minutes later Dr. [redacted] came out of the back area, and he stares at me so I stared back.  Finally, he tells me that [redacted] wasn’t there.  I told him why would [redacted] tell me she was going to call me today, knew I was coming in today, and all of a sudden now you’re saying she is out???  He said I don’t know why she would tell you that.  I told him to call her on her cell phone and ask her how much the refund would be.  Dr. [redacted] told me “no” he wasn’t going to call her on her cell phone.  I asked him why he was not refunding my money since I didn’t cause this—it was Dr. [redacted] who resigned.  Dr. [redacted] said I don’t know how much you will be getting refunded since [redacted] is handling it.  Since I had been in there the day before I yelled at him and told him yes you do know because you discussed it with her before and after I left.  I was angry at this time because I had been going back and forth to Ohio Spine and Body on 10/13, 10/17, 10/20 and 10/21.  He told me “too bad”, I wasn’t going to get my refund, he wasn’t going to call her on her cell phone, and to come back when [redacted] would be there on Wednesday, 10/22 to get my refund.  I went out to my car and waited because I was so angry.  I went back inside and picked up a business card and asked Dr. [redacted] what was her name (I never had [redacted]’s name until 10/21) Dr. [redacted] told me her name was [redacted], I wrote it on the business card and told Dr. [redacted] that I would be back in on Wednesday 10/22 and he had better give me my refund or I was going to call the police.  I also asked him how he could live with himself being a liar and a thief. Yes I did yell this at him.  There were no patients in the waiting area—please see office hours in #6.
Please note:  I left and went to my car, I called my husband on my cell phone—told him what happened, and asked him to call Ohio Spine and Body to make sure that [redacted] would be in on 10/22—I had been given the run around so many times—I wanted to make sure she would be in.  He called, and was told [redacted] was schedule to be in at 9 a.m.  When my husband called me back and told me, I asked him if they told him whether [redacted] was going to be in the entire day.  He said, they didn't tell him.  I asked him to call them back to inquire her workday schedule.  He called them back and the receptionist told him [redacted] would be in the entire day on 10/22.  My husband left his name and phone number.  [redacted] never called him back.  I have the phone record to prove my husband called twice.

The final interaction resulted in the police being contacted and a complaint being reported by our clinic director.
[redacted]'s response: This statement by Ohio Spine and Body is false:  Yes, who wouldn’t be combative—please see definition regarding combative (adj.) from the “Free Dictionary by Farlex” Inclined or showing an inclination to dispute or disagree even to engage in lawsuits, a style described as abrasive and contentious; a disputatious lawyer.    
Again, please see each day/time I came into Ohio Spine and Body:
Monday 10/13 and Friday 10/17—Met with Dr. [redacted] at 10 a.m. both days--calm conversation.
Monday 10/20 --Met with Dr. [redacted] in side office—Asked him calmly to call HR to get my refund—which Dr. [redacted] agreed and went in the back room to call HR (door was closed no patients in room)—later [redacted] came and asked me to come into the back room—she was rude and interrupted me and I yelled and told her to shut up.  I told Dr. [redacted] and [redacted] I would be back on 10/21.
Tuesday 10/21— Came at 12:15—no patients were present (see office hours below), both [redacted] and Dr. [redacted] were told I would be in.
Wednesday 10/22—Came to Ohio Spine and Body to get refund at 12:30 doors were locked—no patients were present.  I knocked on the door, Dr. [redacted] let me in.

Monday:  9a.m. to 11:30a.m.,        2p.m. to 5:30p.m
Tuesday: 8a.m. to 11:30 a.m.,         2p.m. to 4:30p.m.
Wednesday, 9a.m. to 11:30 a.m.,   3p.m. to 5:30p.m.
Thursday, 8a.m. to 11:30 a.m.,        2p.m. to 4:30p.m.

On 10/13 and 10/20 I met with Dr. [redacted] and [redacted] behind closed doors.  When I came in on 10/21 and 10/22 what patients were disruptive since I came when no patients were scheduled (see office hours above)? The doors were open except on 10/22 when I told them on 10/21 I would be back on 10/22 to get my refund and if I didn’t get the refund, I would call the police.  
Dr. [redacted] had the doors locked when I arrived at 12:30p.m. on 10/22.  I knocked on the door and stood there for about 5 minutes.  Dr. [redacted] came and opened the door.  I came inside and he asked “what do you want in a sarcastic tone.”  I told him calmly I was there for my refund since he told me to come back on 10/22.  He said [redacted] wasn’t there (again). I told him I would wait.  He stated she was not going to be there all day.  She was in Cincinnati.  I told him I would wait while he called her on her cell phone in order for me to get my refund—no yelling occurred.  He said if you don’t leave I am going to call the police.  I told him go ahead because I was going to call the police too.  Dr. [redacted] called the police.  I also called the police while in the lobby of Ohio Spine and Body.  The female dispatcher told me to leave because it was private property.  I explained to the dispatcher that I had been back to Ohio Spine and Body numerous times trying to get my refund, and Dr. [redacted] wasn’t refunding my money.  She said she understood and suggested I take the matter to the small claims court, but I was to leave.  I immediately left and went directly to the small claims court on Wednesday, 10/22 to file a claim.  I had to pay $78 to file the claim.

I copied a part of Ohio Spine and Body response to the Revdex.com complaint I filed 10/22: The situation was handled professionally and respectfully by our staff, as would be expected by a clinic that specializes in customer service and attention to patient care.
[redacted]'s response: This statement by Ohio Spine and Body is false:   Seriously???  How can they make the statement professionally and respectfully??  Why was I not given my refund when Dr. [redacted] told me 2 times to come back, 2 times I communicated with [redacted] and Dr. [redacted] I was coming back the next day, and 2 times I was told I would be called to let me know the amount of the refund?  Please explain how this is considered respectful and professional treatment.

However, we will continue to process any refund that may be applicable in accordance with our office policy.
[redacted]'s response: This statement by Ohio Spine and Body is false: I am requesting $400 refund from Ohio Body and Spine.  Originally, I would have settled for $200—this is the $99 per week for a total of two weeks I wasn’t able to complete the program.  I can’t use the 8 lipo injections if I can’t have the phentamine or a physician to discuss my weight loss concerns.  But since I had to make a total of 4 trips back and forth ($12 for mileage each day), go to the small claims court and pay $78, I figured the extra gas, my time, mistreatment, and frustration on not getting my money refunded--Ohio Spine and Body should repay the full amount--$396 plus tax.
 
The bottom line is I paid for 4 weeks of a weight loss program and because of a situation out of my control, and after coming back to Ohio Spine and Body numerous times, left each time without a refund.  It should never have come to me having to file a claim to the Revdex.com or go to a small claims court since a refund was confirmed by Dr. [redacted] to be credited through a quick swipe of my credit card.  
Ohio Spine and Body’s policy is to pay for the program before starting it and using my credit card resulted in a transaction in a matter of seconds.  Since Dr. [redacted] and [redacted] stated they would credit my credit card, the transaction would have involved a matter of seconds to perform thus providing Ohio Spine and Body the opportunity to practice the “specialized” customer service and “attention” to patient care as stated above—but unfortunately they did not do so at all—very disappointing.

We are in receipt of...

the complaint submitted by our previous patient, [redacted]. 
 
The complaint states that the patient paid for a 4 week medically supervised weight loss program in our clinic on 9/29/14 and was not delivered the program she had paid for.
 
While we do agree with the fact that her treatment plan was not completed as scheduled, the details of the situation as described by Mrs. [redacted] are in fact, not accurate.
 
The medically supervised weight loss program is directed by our medical physician. This physician left the clinic unexpectedly and without notice on October 9th due to a personal issue. At the time, we were led to believe he would be returning to the clinic and this was only a temporary issue.
 
We rescheduled the patients who were being seen by this physician to the day we expected him to resume his usual schedule. The physician then did not return to the clinic and did not communicate as to his reason for not returning or even notifying us in advance of his intentions to not return.
 
In Mrs. [redacted]'s case, we attempted to explain the situation and indicated that she would have a credit and/or refund of any items that were not provided to her, as part of her treatment plan.
 
Mrs. [redacted] had been given all items prescribed in her plan with the exception of a 2 week course of appetite suppressant medication. This medication can only be delivered to patients by and under the direct supervision of a medical doctor.
 
Once she was informed we were not able to deliver the rest of this medication, Mrs. [redacted] became combative and insisted she receive a refund immediately. The dollar amount she has requested has changed from $200 to $400. The original package purchase price was $396.00.
 
It was explained that we would have to reconcile her account to determine if there was in fact a refund due and what that actual amount was. The financial policy that was signed at the time she started her treatment clearly establishes a refund policy. All refunds are processed off site at our administrative office and paid within 30 days of the request by check.
