In mid May I decided to make arrangements to be cremated when I dieI went on line and found Woodlawn Funeral Services in Seymour associated with Legacy.com which seemed to be a good price for me financially as I only wanted the simple cremation and no frills, no service, no flowers etcas my remains will be taken to Florida and scattered in the Gulf of MexicoI am an artist and designed my own container to hold my ashes that was compliant with the law and completely biodegradable I tried to fill out the form on line but the site does not allow you to bypass options you may not want ie: purchasing a Coffin or Urn, flowers, showing, etcso I called the Funeral home and told them I was not able to fill out the forms due to website designWas told by Kendra that forms would be mailed to me and I could return them by mail and that the web site option problem would be addressed It is now July 1st and have not received the formsCalled Woodlawn again and spoke to Cindy I was told they will be sent (again?) and should be able to fill them out on line also tat the problem has been taken care of I went on line and filled out the form and submitted it July 24, I received an email from Woodlawn (Andy R***, owner) confirming receipt of the form and was told "Will be in touch" Now it's Sept.30th, and have not received any further emails or any phone calls or phone messages regarding my plansI again notified via email, Woodlawn I had not been contacted it's now October 1st.) I received an email from Kendra stating that she had called and sent emails and had not been able to contact meLike I said I hadn't received anything further form them until this email in the past weeks and I am home almost all of the timeI decided not to deal with them because it doesn't take 1/months to make arrangements no matter how "Busy"(excuse given) they have beenI would have been a customer too but not nowI will make arrangements with a trusted Funeral Service I've dealt with in the pastEven though it may cost a little more it will be worth itI believe because I was not having a traditional expensive funeral my request had just been put aside then ignoredI probably should file a formal complaint but I think this review will get the point across to treat everyone with the respect and their wishes in a consistent timely manner!

Business Response to a ComplaintComplaint ID#:***Company Name:Paul Mitchell SchoolCompany Contact:MICHAEL S*Company Phone:***Company Email:***Person Who Sent the Complaint:*** ***Staff Member:*** ***Response:Please know that I have been out of town (Pittsburgh) the last several months due to a severe, on-going family emergencyTraveling back and forth for brief periods of timeI am in receipt of all the Revdex.com correspondence and please let the customer/complainant know that we will issue a full refund of the amount paid for the gift card this weekWe have the complainants name and address as: *** *** *** ** *** *** *** ** *** *** We were not aware of the Gift Card Act, as I gift card policy was one yearWe apologize for this inconvenience

I am in receipt of your letter dated October 22nd, that has an official post mark date of October 23rd, 2017. The assigned ID# for this case is:***. The Customer's name is: *** *** My business's name is: Paul Mitchell The School - Jersey Shore and my name is:
Michael JS*** Please know that I fully understand, respect, and appreciate your position in this matter. My position regarding the letter is that I strongly disagree with the statements that were provided to the Revdex.com and yourself from the "Customer", *** *** *** And, I will list those disagreements below. *However, I must first point out that the business in question which again is my business, Paul Mitchell The School - Jersey Shore is a career training school that is an accredited institution of higher education which has obtained its accreditation from a nationally recognized accrediting agency (COE), we are approved by the Federal Department of Education to participate in our governments Title IV student loan program, and we are licensed by the New Jersey State Board of Cosmetology & Hairstyling. These three (3) different agencies along with our franchisor, Paul Mitchell Advanced Education, LLC are our School's governing bodies that our School must maintain strict compliance with each and every year to maintain our School's eligibility to operate. One specific area of compliance is the Federal government's FERPA Law. As you probably already know FERPA stands for; The Family Educational Rights and Privacy Act of 1974 is a United States federal law that governs the access of educational information and records. All educational institutions that receive federal funding must comply with FERPA. Because of the FERPA Law I am restricted to the information that I can provide at this time unless the student is willing to sign a "Release of Information" and provides me his or her permission to provide such information to you. At that point I would be more than happy to provide factual evidence that strongly contradicts the "Customers" assertions. My first area of concern is that neither I, Michael JS*** - or - Paul Mitchell The School - Jersey Shore has technically or legally conducted business with M** *** *** Neither *** *** nor his wife where willing to Co-sign the School's student Enrollment Agreement at the time their child enrolled in our School. It is important to note that the student was above the legal age of in order to enroll on their own. *However, the School does have "Release of Information" forms on record or file for the parents of the student and the School has previously supplied ALL documentation to the parents and the student, so they could easily forward that documentation to you as well. There is a "Purchase Date" listed of: 12/16/2016. *This date is incorrect as the Enrollment Agreement was signed on 10/16/2015 and the class commenced on 10/19/2015. This is a discrepancy of yr and two months time.States the "Problem Occurred" on: 02/10/2017 *The student was not even enrolled in our School anywhere near this time. (*I really would love the opportunity to provide you with all the documentation about this scenario, so I hope that the student elects to provide the "Release of Information" form that lists your name.)The "Customer's Statement of the Problem" is 100% as the education was not completed. There is, in essence a two part requirement to be considered "complete" or a "graduate" from our School. The student not only has to finish their 1,ACTUAL hours of being in attendance but they also have to complete all the Academic requirements in order to be considered "complete" or a "graduate" of the School. (**Again, I would love to be able to provide you with much more detail but I must refrain until I have received the "Release of Information" or, I receive further advice from legal counsel that may allow me to provide such information when a situation like this arises. The "Customer" provides a "Purchase Price" of ($23,500.00) and a "Disputed Amount" of ($23,500.00): The cost of our program back in was broken down as follows:Application & Registration fee $ 150.00Student Kit/books/lab coats/etc. $ 3,000.001,200hour Tuition @ $12.00/hr $ 14,400.00TOTAL COST $17,550.00 NOT $23,500.00. That is a difference or overstatement of: ($5,950.00)*I will tell you that our School went ABOVE & BEYOND to do everything that we could in this situation to help. And, our efforts of going above & beyond didn't just recently begin. There is real factual documentation going way back that clearly shows a consistent history of our School and team members literally going above and beyond to help. **And, even through a time that one of the parents chose to not listen or read throughany of factual evidence and was extremely disrespectful to one of our team members our School still continued to do the right thing and ALWAYS kept the students best interests our #priority.I look forward to receiving your response. Hopefully, you will have been able to obtain the "Release of Information" from the student. And, I will continue to provide any and all documentation that I am allowed to provide. Please feel free to call or email me at anytime.Sincerely, Michael JS***, CEO*** Cell/text/anytime

In mid May I decided to make arrangements to be cremated when I die. I went on line and found Woodlawn Funeral Services in Seymour associated with Legacy.com which seemed to be a good price for me financially as I only wanted the simple cremation and no frills, no service, no flowers etc. as my remains will be taken to Florida and scattered in the Gulf of Mexico. I am an artist and designed my own container to hold my ashes that was compliant with the law and completely biodegradable.
I tried to fill out the form on line but the site does not allow you to bypass options you may not want ie: purchasing a Coffin or Urn, flowers, showing,
etc. so I called the Funeral home and told them I was not able to fill out the forms due to website design. Was told by Kendra that forms would be mailed to me and I could return them by mail and that the web site option problem would be addressed.
It is now July 1st and have not received the forms. Called Woodlawn again and spoke to Cindy I was told they will be sent (again?) and should be able to fill them out on line also tat the problem has been taken care of.
I went on line and filled out the form and submitted it.
July 24, 2016 I received an email from Woodlawn (Andy R[redacted], owner) confirming receipt of the form and was told "Will be in touch"
Now it's Sept.30th, 2016 and have not received any further emails or any phone calls or phone messages regarding my plans. I again notified via email, Woodlawn I had not been contacted.
it's now October 1st.) I received an email from Kendra stating that she had called and sent emails and had not been able to contact me. Like I said I hadn't received anything further form them until this email in the past 9 weeks and I am home almost all of the time. I decided not to deal with them because it doesn't take 4 1/2 months to make arrangements no matter how "Busy"(excuse given) they have been. I would have been a customer too but not now. I will make arrangements with a trusted Funeral Service I've dealt with in the past. Even though it may cost a little more it will be worth it. I believe because I was not having a traditional expensive funeral my request had just been put aside then ignored. I probably should file a formal complaint but I think this review will get the point across to treat everyone with the respect and their wishes in a consistent timely manner!

I am writing this email in response to complaint ID#:  [redacted]Disclaimer:  I do not want this email or any part of this email published in any manner.  In the event that you require a response to be published, I will draft that response.I apologize in the delay in getting back to...

you, however I was unfortunately out of town due to a extreme family emergency and have returned to find several of your letters. The makeup program that [redacted] enrolled in was a (8) week (3days/wk - 4hrs/night) makeup program beginning on November 10, 2014.  The program costs are:Registration fee =      $   100.00MAC makeup kit =     $   900.00100 hour Tuition =     $ 2,200.00 Total Cost =    $ 3,200.00[redacted] enrolled in our November 10, 2014 makeup program in September 2014.  At that time she paid a registration fee of ($100.00).  She then paid for the cost of the kit ($900.00) on October 13, 2014 which she received when she started the program.  There was a total of (5) monthly payments of ($440.00) to be made on or before the 10th day of each month.  The first tuition payment was received on December 9, 2014 and the second tuition payment was received on 01/02/2015.  These were the only two tuition payments made.  Totaling ($880.00) or 40% of tuition costs.[redacted] made a complaint about the class size being 22 students.  The enrolled class size was 18 students, 25% smaller than the typical and/or widely accepted class size of 1:24 (instructor to teacher ratio).   The average actual in attendance class size was 14 to 18 students.[redacted] did make a complaint at the very beginning of the program and I myself attempted to meet with [redacted] about her concerns.  At this time (early December) [redacted] informed her instructor that it wouldn't be necessary and everything was okay.If the program was not of value and education to [redacted], I am not sure why she continued attending the program.  [redacted] completed the program and participated in the programs photo shoot.   The photo shoot provided her along with the other students a professional photographer to shoot her models for a day and evening look.  The enrollment agreement that [redacted] signed contains a tuition reimbursement schedule in the event a student were to leave the program for any reason.  The tuition reimbursement is as follows:Percentage of Time Enrolled                           Amount of Tuition Owed or Retained by SchoolLess than 5%                                                                       10%Less than 10%                                                                      20%Less than 15%                                                                      30%Less than 25%                                                                      40%Less than 35%                                                                      45%Less than 50%                                                                      70%50% and over                                                                      100%This program has been offered and attended for years with a lot of extremely happy graduates.  This is the first complaint of this nature that I have received pertaining to the makeup program.  It is essential that the program treats each and every student the same.[redacted] is free to call or email me, as I would be happy to speak to her and even meet with her about the complaints she has made.Regards,[redacted]
