I am rejecting this response because: no one has contacted me at this current timeI’ve left messages with corporate in CA I had no issues with the dentistThe manager took it upon herself to dismiss my case without notifying meI was no comfortable with the manager billing due to she made two mistakesI politely asked to have a mother representative to bill or speak directly with the dentistThe office assured me that was ok and to come in for the next appointment on the 4th of NovemberI then get an email stating I’ve been dismissed without discussion or reason whyI now have to wait months for another dental appointmentI was scheduled to have a root canal and crown on the 18thIf something goes wrong or I lose my tooth I am going to take legal action against your companyI have documentation as well disputing your claimsAgain I have heard from no one at peak dental since posting this complaint.

Pt's first visit with our office was October 30, for treatmentPt was set up on monthly payments inthe amount of Finances are always gone over and set-up, prior to any treatment services beingrenderedServices were completed and ptpaid her out of pocket over months.Pt's second
visit with our office was for her six month follow-up/ routine check-upPt was seen on04/30/for this appointmentPt was given a treatment plan for a lower partial, and two crowns onthe upperI spoke with ptand told her that she may utilize our in office financing, because she wasdiligent about her past contract with usPtagreed and scheduled for treatment on /16/2015.When, ***, came in she was brought back to start treatment before the contract was set-upPt.came up front when services were completed for the dayPt was told she would either need to pay thecontract in full that day, or we need to have a card so the monthly payments can be drafted, just as wedid with her last contractPttold us she wanted the FSA card to be used, but she did not have it at thattimeWe explained to the ptthat we can't take her word for her payments that we have to set it up thisdayPtstormed out of the office, did not pay anything that was owed and ignored phone calls when wetried to reach out to her for payment.Total cost of services ptis:D(ceramic crown) - $900.00, ptis responsible for 50% of this feeTooth #& #were preppedthis dayPtis responsible for this amount at the time of service; ptdid not pay for this prior to leaving.D(lower partial) -1079.00, ptis responsible for 50% of this fee, leaving her portion andthat divided into four visits is Pt also left before paying this portion.Total out of pocket for pt.'s treatment was $1439.50, plus her deductible of $50.00; leaving a grand totalof $1489.50.Pthad two crown preps and impressions taken for both of these servicesAs our company policy, we donot render services for anybody without a form of payment; this did not happen in *** situation.Pt made a $payment from her FSA, on 11/04/This left a balance of 133.88, this is thebalance owed to us for the lower partial impression.We have incurred lab fees for all of the services renderedPtis responsible for Please feel freeto contact our office with any questions or concerns.Thank you,Jessica J***Office ManagerPhones: 425.786.9702Fax: 425.747.2570Email: ***@westcoastdental.com*Please view attached document
