We require that proof of residency documents are solicited in person to protect the personal information of our patients and to deter from fraudThere have been many occasions when separated parents come to request a proof of residency with the intentions to claim their children which have caused
us much grief with the IRSWhen the document is solicited we verify if the insurance of the patient is active and the coverage of benefitsThe patient is then informed in person whether their insurance covers non-medical documentsSome medical insurance do not cover non-medical documents; therefore we are in our right to charge a fee for the proof of residencyIn the case of *** *, her medical insurance does cover non-medical documents and would have not been charged a fee after her insurance was verified in personWe valued our clients and we hope that *** * can forgive the miscommunicationWe regret the inconvenience and will gladly provide the reque sted proof of residency when the client comes to our office and solicits the document
