I had a claim in February of that was billed and paid through my HRA account from ***The check was sent to Pennsauken and cashed in March of In August of I was notified by a collections agency, ***, that my bill was unpaid and was in collections I, along with ***, my Employer and a mediator worked on getting this issue resolved with the collections agency and Pennsauken Because of the inability for Pennsauken billing to apply the fund to the appropriate account and work with me to get this proof of payment communicated to the collections agency, I ended up getting the collections reported on my credit report Pennsauken accused me of not paying, never sent me any notice that I was delinquent on my account, according to their records, and never notified me until *** notified me that my bill for $was sent to collections This started in August and is now just getting documented, but the derogatory statement was added to my credit report and i

Review: I was seen 12/28/13 for an MRI, in advance of the service date and the day of I was told no additional fee from me required. Many months later they billed me $63.75, which O PIAD in full and have canceled check. PDC via [redacted] entered a false collection account to my credit profile of $128.00 twice what I was billed and that I paid. I was altered to this via my credit monitoring agency. I have a clear credit profile with no unpaid, late or collection items as evidenced by my July 2014 credit report.PDC has refused to remove this fully erroneous item. This has effected a clear credit profile and credit score putting me in jeopardy of increase fees and costs.Desired Settlement: this item fully and completely removed from my credit profile. [redacted] is the Medical Billing Office for Pennsauken Diagnostic Center located at [redacted]. We are responsible to bill the insurance and then the patient as per the claim gets processed.

For Mr. [redacted], date of service 12-28-2013 - MRI LOWER EXTREMITY W/O DYE was billed to [redacted] insurance and [redacted] processed the claim and made a partial payment and applied $63.75 towards Mr. [redacted] responsibility. We sent out 2 patient statements to Mr. [redacted] on 02-17-14 & 03-18-14 but did not receive any response until 04-23-14 and hence forwarded this account to a third party Collections Agency "[redacted]". Letter from [redacted] went out to Mr. [redacted] on 4-28-14 with no mail return. The letter instructed Mr. [redacted] to make payment of $63.75 to [redacted] and provided an envelope in which to send the payment to [redacted]. As no response came from Mr. [redacted], the account was then credit reported for $63.75 on 7-15-14. In between, Pennsauken Diagnostic Center received a check dated 06-12-14 for $63.75 directly from Mr. [redacted] and forwarded it to the billing company and we posted the payment amount and closed the file around 06-16-14. Mr. [redacted] was supposed to send this check to [redacted] in the envelope they had provided so that they could take off him from further collections action, but instead he sent payment directly to Pennsauken Diagnostic Center.

Now, we have already let [redacted] know about this and they have already taken action on 09-03-14 to instruct the credit bureaus to remove this item completely from Mr. [redacted]'s credit reports. It will take 45 days to get this item completely off his report.

We regret this happened and has caused inconvenience to Mr. [redacted].
