February 11, 2016Dear Revdex.com,It was brought to my attention on 2-10-that a current patient filed a claim stating that our company is taking advantage of consumersIt is, in no way, our policy to do such, nor has it ever been our policy or practice to take advantage of our
customers and or patients.On September 16, this patient received a *** *** W.T.O(Wrist Thumb Orthosis) from a provider at the *** *** *** *** *** *** office in Hazleton PAAt the time of service this patient signed the following forms for release of the product:A signed delivery ticket stating that she was receiving a new productAn ABN of Non-coverage that in the event that Medicare would not pay, that the patient would become responsible for the payment of the equipment receivedThis is a standard form for our Medicare patients, due to the changes in regulation that often occur in coverage items.On October 22, our office received an EOB (Explanation of Benefits) from Health America Pennsylvania, incthat her insurance would NOT cover the above items and that the billed amount would be the patients responsibilityOn November 2, we sent the patient a bill in the amount of $for the above orthosis that she received at her doctor's visit on September 16, We made several attempt to call the patient to discuss this bill and how we could assist in the payment of this billUpon making several attempts to call this patient we were not successful in reaching her to discuss her billThe telephone number that we were given was ###-###-#### which was not in serviceWe then sent out our day notice to the patient on 12-07-which clearly stated that is was her "second notice"On December 20, We were able to make contact with patient to discuss this billWe agreed that we would discount the bill by $and that she would be responsible for only $to pay for the orthoticThis was due to her being on social security and financial hardshipShe agreed verbally over the phone to the previous payment and discountOn January 5, the patient sent us a check in the amount of $for payment of her outstanding balanceHer check number is *** from account number *** paid to the order of Performance Biomedical IncOn January 6, she received another billing statement regarding her account and it reflected the $dollar payment made on the 5th of January and "thanking her for her recent payment"this is standard policy for all our patients that have Outstanding balances due to copays and deductible'sWe make it our policy to call and settle payments less than what is billed to the insurance agency and most patients find is very helpful and cost effective for them to have a lower bill, considering the high deductibles that many of our patients face with insurances.On February 2, this patient received a letter from our office stating that she would be sent to the magistrates office if payment was not collected by February 15, in the amount of $Which prompt a response from the Revdex.comOn February 10, we were made aware of an accidental letter that was sent out in regards to this situation from our accounting teamWe have corrected this error and the few patients that received this letter, in error, to disregard the letter and that it was our mistake and to continue with the agreed upon payment arrangements that were made in the pastWe also called the patient in question regarding this letter and apologized for our mistake, and stated that she could continue to pay the agreed amount that was previously discussedDuring this conversation she was unwilling to cooperate with our billing department and stated "that it was in the hands of the Revdex.com and she was not paying the agreed upon amount"Please understand that we are within our legal grounds to pursue collecting the entire amount owed and that we demonstrated great patience in allowing this much time to lapse during this payment processIt truly grieves us regarding this issue and we would like to have had resolved this issue at the lowest level possibleWe will continue to pursue obtaining our payment from this patientWe apologize for the letter mistake, but that doesn't excuse her from her obligation for payment for product that she received from our officePlease feel free to contact me regarding this issue and would like to assist in helping this patient, as well as all our patients, not suffer the financial burden that is associated with co-pays and deductibles.Thank you, Ronald C.CEO

Dear Revdex.com,In response to a message I received I never willingly signed or was aware I was liable for payment of brace All I was told was most insurances cover it! I was never aware either that Medicare paid $ With my $that makes a total of $or $if I would pay the $initially asked for which is totally outrageous!! Not until I talked to a lawyer after I received the letter which was "mysteriously" sent by mistake was I contacted again & told they have to get their money back somehow! It seems the Medicare & my payment should be sufficient I hope this matter be resolved since it really is affecting me all for $25, seems a bit ridiculousSincerely,

Dear Revdex.com,In response to case number [redacted]. As a company it is always our responsibility to take the Customers concerns seriously. We make every effort to resolve issues, including payment options, at the lowest level possible. The facts to this case are as follows:1. [redacted] received product from the orthopedic office in Hazleton, PA2. [redacted] signed the document stating that if the insurance didn't pay, she would be responsible for payment.3. [redacted]'s insurance that we have on record is [redacted] (Not Medicare) (See Attached). [redacted] replaces Medicare. Medicare does not pay.4. We sent three notices, one a month, explaining what [redacted] owed. [redacted] did notcontact us until November 2015 and did not send payment until January 2016 which was $25, as documented in my last letter.5. We also tried calling her and the phone number was not in service.6. We were contacted by her nephew on Monday, February 22, 2016 in which he stated that he was a lawyer in the Western PA, but didn't state that he was representing her. He wanted an explanation of the bill to work it out. We advised him to fax over signed statement from the patient to give us permission to speak to his office. Which we have not received as of today.7. When we spoke to [redacted] in February upon receiving the correct phone number fromRevdex.com, we stated that the Medicare allowance is $212.00 dollars for the payment of the brace. During that call we tried to resolve this invoice and negotiate a comfortable amount, but she advised that she will not pay anymore, since "people" told her that since its health insurance; she doesn't have to pay anything.Performance Biomedical has done everything to resolve this matter. We cannot do anything more to resolved this matter. We hope that this case will be closed by Revdex.com completely. It is up to the patient to call our office and resolve this issue regarding her account.Thank you,Ronald C.CEO
