02/20/2017Dear Cynthia,We did bloodwork for [redacted] on 6/23/All tests were run at Perll Diagnostics, Incexcept for the Lyme Antibody which was referred to [redacted] .We submitted a claim for the tests run at Peril Diagnostics Incto ***'s insurance which at the time was [redacted] We did not charge for the test referred to [redacted] .Loomis sent us an explanation of benefits (EOB attached) which shows it paid partially and shows a patient (member) responsibility of $We discounted the owed amount and billed [redacted] for $82.20.We expect this balance to be paid.Thank you.Sincerely,Nava NM.DFCAP Laboratory Director
