RE: Complaint ID [redacted] Dear Ms [redacted] I am writing this letter in response to the complaint filed against our company, in an effort to resolve the dispute and address the issues presentedSince Ms [redacted] has requested your involvement in the dispute, I am able to disclose medical information in direct response to her issues, but I am not authorized to disclose any additional medical information without a written release from the patient outside of this dispute.According to the information provided, the customer’s statement was “My compounded prescription of [redacted] / [redacted] was INCORRECTLY labeled ‘take one capsule daily by mouth’ leading to an extremely HIGH level of estrogen in my body (which can cause cancer)It was supposed to be labeled ‘take ***’”I will address each of those areas below in response to her complaint.Ms [redacted] was previously a patient with R [redacted] **That company ceased operation and subsequently claimed bankruptcyIn an effort to continue to provide service to those patients, our pharmacy was forwarded requests for patient refillsWe were not able to obtain a valid transfer from [redacted] s files or their personnel, therefore a new prescription order was requested from the prescribing physicianThe new order was dated 12/17/and received via fax on 12/22/Upon receipt of the order, there was a need for clarification from the prescriber, due to the fact that [redacted] and [redacted] are not standard pharmacy abbreviations and the directions were difficult to readOn 12/30/15, Regina W [redacted] PharmDreceived verification from the physician that the prescription was for oral capsules, which was documented on the hard copy of the prescription orderThe prescription was therefore dispensed to the patient for oral capsules upon clarification from the physician, with instructions to “Take one capsule by mouth daily as directed”.Ms [redacted] was charged $for her first order of the medication in question on 1/7/(copy of invoice included)As far as I am aware, Ms [redacted] did not contact our pharmacy or our corporate office with a complaint regarding the change in the administration route of her medication and requested a subsequent refill that was dispensed on 2/9/We dispensed the medication as specified by the physician’s office, and would have been more than happy to address any concerns about the medication or directions with the patient and her providerWe strive to provide quality care, that is safe and effective, to all of our patientsI do understand the concern the patient has with the change in administration directions from her previous pharmacy to ours, but I am not in agreement that it is a result of negligence on the part of our pharmacy or pharmacistsI do not feel that we are obligated to refund the payment for the prescription.I would like to briefly discuss the statement that the oral intake of the capsule led to increased levels of [redacted] in her body, which may cause cancer, versus how she had previously been directed to administer the medication (which was not specified)According to the package inserts for FDA approved commercial products containing [redacted] : the increase in [redacted] / [redacted] ***/day is comparable to the oral absorption of [redacted] and the bioavailability of [redacted] after [redacted] administration is approximately 56%, which is comparable to that of oral administration ( [redacted] )Therefore, the oral consumption of [redacted] versus [redacted] or [redacted] administration would most likely lead to a comparable or lower blood level of [redacted] .I appreciate your company’s dedication to maintaining quality standards of consumer goods and servicesPharmacy Innovations strives to provide quality products, genuine care, and exceptional customer serviceAs a pharmacist and employee of Pharmacy Innovations, I would feel a professional obligation to refund a patient’s fee if we provided an unsatisfactory product, or were negligent in performing our professional dutiesHowever, I do not feel that this patients dissatisfaction is a result of either of those factors.If you have further questions or concerns, or would like discuss this matter in greater detail, please feel free to contact me at the business phone number providedThank you for your time and consideration.Sincerely,DrKelly *G [redacted] , Pharm.D.Pharmacist-in-Charge Pharmacy Innovations-Erie

I enclosed a copy of the prescription and it is clearly legible and I dispute that it was difficult to read.Pharmacy Innovations is irresponsible, taking zero responsibility and claims to not know compounding terms.I am not letting this go [redacted] Cc: [redacted]
