Phoenix Choice

Attached is a copy of the appeal I sent the Phoenix Choice the first week of February I have not yet received a reply I do not even know why they are not paying this claim asthe have never sent me anything in writing I really need help with this, not just another person to contact What is the problem, and who or what needs to be done to correct it???Procedure was to be paid at 100% Their website does not give to ability to view claims!!!
