*** See Attached *** *** *** was seen at our clinic on 10/26/for an eye exam that was submitted her BCBS per her request(see attached signature agreeing to our financial policy)BCBS processed her $claim and made no payment and assigned $toward her deductible
The remaining balance $was assigned as an insurance discount that was written off her account*** was mailed four statements (11/07/16, 12/20/16, 01/17/17, 02/24/17) with no response or payment madeEven after we mailed a Final Notice sticker attached to her statement on 2/24/notifying her that her account was going to be sent to collections, she still made no payment and didn't contact our office regarding her overdue balance***'s account was submitted to collections on 03/23/for $which included accrued finance chargesShe has an extensive history of unpaid balances, and I have included a copy of all notes regarding her history of unpaid balancesPine Eye Associates is very confused as to why she would have any basis for a complaint regarding our clinic as we provided services per her request, we submitted her claim to her BCBS policy per her request, and can't be responsible for the fact that services rendered weren't paid for by her insurance and assigned as her responsibilityWe will not release her prescription because she has never paid for services that were renderedSincerely, DeAnn *** Billing Department
